Let say you were out minding your own business until some weirdo (look like a mad scientist) come up to you. This scientist want you to fill out your details and signatures on this form. The scientist explain that you are the perfect test subject for his experientist (didn't say what it is).
No matter what you do or say, he won't take no for a answer so you decided to fill it in with false details just to get the scientist to leave you alone. Once you done with the forms, the scientist is happy and leave you be and continue to what you were doing before the weirdo show up.
Sometime later you are dead (died, killed, murder, or however you want). You are now end up to whatever you have believe in when your died. Just when you about to embrace it, suddenly some force is pulling you away which you are powerless to resist.
You are now back to the world of the living but in some kind of a chamber. You are fix in one place you are unable to move your head etc. The scientist show up and explain what is going on. It turn out the experientist he didn't tell you was this. The signature is you had agree that once you have died you are willing to come back to the living. The detail is supposely for the scientist to track you down (the scientist is not please that you had lied to him/her).
Anyway you are not in a horrible situation per say. The scientist have given you some will to have a option on what sort of body you want to come back to life as (no the scientist will not end your life to give you rest and peace). The options of the new body for you to be in is listed on the poll.
Here is a quick explaination to what the body in the poll are-
Robot- Robot apperances with metal skin etc
Android- Human apperances (but look fake) but with robot body parts on the inside (look like you or someone else)
Clone of you- A clone of you which is brain dead until you enter it. The apperance is either what it was right when you had died or can be years younger or older you want it to be (the scientist got hold of your DNA before you died).
Empty human body- Same as the clone but it is not your apperance. Its apperances can be anything you want it to be (gender, ethnic race etc)
Zombie- Yup if you chose this, you will be mindless and crave for flesh/brain. The zombie body is what left of your decompose body or if you had died with no body left e.g. you died from falling into a lava pit (which the scientist got hold of your spirit/ essense), the scientist provide you a random corpse.
Undead Minion- Same as a Zombie but you got free will to some limits since you are now serving a master (the scientist or someone else) and do your master bidding much to your dismay.
Frankenstein- Yup the scientist had build a single person together using different body parts.
Another humanoid body- Whatever you want but doesn't fit in the rest of the poll. Its got to be a humanoid body/ shape so no you cannot be in a animal/ creature like body.
