Yes. I've also seen the Plasma Rifles of Elites overheat in Halo games. Reloading happens, you just don't notice it all the time. The most obvious case is with the snipers in Gears of War, who need to reload after every shot.

Fallout 3. Enemies reload, which is a perfect time to let loose, in VATS or free-aiming.

Ironically (considering your Avatar), Fallout 3. With my mod, they use ammo too, which can be a *****, but it is more realistic.

I have, but I wish more of it was based on how much ammunition they actually have, rather than to give you a break.

Yes. I'm pretty sure in Rainbow Six they shout "Cover me! I'm reloading" So it's pretty easy to tell when to pop out from cover and shoot the guy. Unless the sound doesnt trigger at the same time and hes already reloaded then he shoots you in the face.

Pretty much this. You can actually see them reloading in R6:V.
