Pretty much whenever a squadmate/former squadmate died in Mass Effect 3. Mostly Mordin and Thane.

Just about everything in The Binding Of Isaac.

That game has 14 endings and the one where Isaac hangs himself is still probably the happiest one.

Slowly walking toward the light.

Agro falling to his death after having stood by me as a loyal companion throughout the game.

Bioware games have never been emotionally engaging for me at all. I can't get connected with any of the characters in a meaningful way. Maybe because they're just combat npcs, with a few romance lines and a meh backstory. You never do anything important with them.

Except Blizz from SWTOR. I would have that jawa's babies.

The only game that made me cry was Heavy Rain. As cold and mechanical as I am, I just can't help but to cry during certain sad things. I cried quite hard during the Where the Wild Things Are movie (yes, laugh, I know). I even cried at the end of LoTR Return of the King where everyone bows to the hobbits (though in my defense many grown men cried during that part). I have a hard time getting emotionally invested in something, mainly from the fact that I'm quite the loner and very few people have ever given 2 shits about me, so its hard to give 2 shits about others.

'It hurts when she... when I remember... me.'

Ouch.

'Okay hero. Set me free.'

'Freya, you say? I believe this is the first time we have met...'

'You're right, but I don't think we build cemeteries for the dead. Sure, it may seem pointless to you, but... How can I describe it? It's so that we can think like this: 'We'll never forget you. We'll remember you every time we stand at your grave. And we won't let the fear of death, which each of us knows, stop us from living our lives. ...Because my friends will remember me when I'm here.'

And just the whole of Fallout 3. Nothing like stumbling across the bones of a child next to a bomb crater to make you get the terrible heartbreaks.

Captcha: it's over. Thanks Captcha, that makes me feel a whole lot better.

Well, time to drag my favorite game out from the shelves again.

<spoiler=PS:T spoiler within>
To say nothing of the entire scene with Deionarra, but there's no cutscene for it - suffice to say, what happened to her is one of the cruelest things any game protagonist has ever done (in the past story, that is). And if you decide to keep up with the sentiment of it in your game...you're a horrible, horrible person. I mean, it's a truly evil thing to do.

Mordin's death in ME3 (oh, come on, this isn't a spoiler any more!)
Yeah it is. ME3's only about 5 months old, hell, I only finished it yesterday. You should really spoiler tag every spoiler, no matter how old.

OT: Since I just finished ME3, mine are mainly drawn from that:
Legion's death (Keela se'lai my friend. *sniff*).
Saying goodbye to Tali (my Shepard's LI) before Harbinger blasts you.
The bit where Shepard and Anderson are just sat looking out at Earth.
John Marston's death.
The end of COD4.

Yes I did. It was a long time ago but I remember that Zero dies at the end and not Big Boss.

Big Boss comes into contact with Snake's new strain of FOXDIE and he goes to sit besides the Boss's grave. He has one last smoke and says 'This is good, isn't it?' then dies at last.

(SPOILER ALERT)
One of the biggest ones for me was the ending of GTA4 where Roman Gets Shot on his wedding day.
(SPOILER OVER)

My number one choice though is when Boss lays Carlos to rest in Saints Row 2.
