I once watched a movie called Life is Beautiful. The whole first half is a fun comedy, then the second half is all about destroying these characters you like so much and methodically crushing your soul.

Futurama with Fry's Dog: I really like dogs, more so than any other animal (not in a sexual furry way though) and cannot stand to see bad things happen to dogs. Coupled with that aspect, I cried quite seriously after seeing that scene.
Clannad during the Alternate reality arc with Tomoyo at their break-up scene. I probably shouldn't have watched it before school because I spent the whole day fighting off tears. I don't know if it was that sad, but something about the scene itself and tomoya's reaction to the break-up as well as hers just made me so freaking sad, it was ridiculous.
That's all I can really think of at the moment. I tend not to watch that much sad stuff and I would've remembered if it had that much of an impact on me.

3 films that made me get a knot in my throat were Toy story 3, The Green Mile and Forrest Gump. Yeah Tom Hanks is in all of my top 3 sad movies list. As for literature a short story I read a while back called The Scarlet Ibys was a tearjerker. I can't think of any musical examples right now though.

A japanese movie called Himitsu/Secret (1999).
It starts off a little bit cheesy. A mother her daughter get into an accident and the mother dies. However the mother's soul gets implanted into the daughter's body. Then it becomes a sort of drama-comedy with the father and the mother/daughter trying to live a normal life together. The mother's soul begins to fade away in the beginning of the third act of the movie and the daughter tries to move on with ther life, entering a collage and finding a boyfriend. The movie ends with the wedding of the daughter and the reveal that
The mother's soul never left the daughter's body, she chose to leave her husband and move on as a new person.
Then the movie just ends.
. It's like being promised chocolates then getting hit in the stomach with a bag of discared valentine's day gifts.

Also "The Late Philip J. Fry" episode. I wonder why nobody ever quotes that episode.

I can't believe so many people think that Fry's dog dying was the saddest episode. Seriously, I see people say that everywhere on the internet. I thought "luck of the fryish" was sadder than that.

The song "Flowers and football tops" by Glasvegas. I knew it about about someone getting killed but I read the Wikipedia page for it earlier and fucking hell

Most depressing thing I've ever seen is probably the first 5 minutes of UP. Ihaven't many depressing films like Requiem For a Dream though.

Back in my younger teenager days the saddest album ever would have to be The City Sleeps in Flames by Scary Kids Scaring Kids. Part of it may have been my age and the things I was going through but it is very dark. I've long ago moved onto much manlier and mature music but holy shit, if you can put aside the general dislike for that type of music that seems to be out there and just listen through that album, it is so fucking depressing. Whenever I hear it now I cringe, it isn't even subtle how it is portrayed, it is just an album about basking in depression.

Most of those are actually bittersweet, but fuck me, they don't half make you feel like shit.

Games: the ending to Mafia and the ending to The Sands of Time. Mass Effect 3 does a lot of endings exceptionally well (and miserably), even if it doesn't do the ending well.

And for other films, Moon springs to mind.

Huepow knew all along this was going to happen. And yet he still tried to save his best friend. By the time Klonoa disappeared and Lephise was starting to finish up her song, tears came out of my eyes.

well let's see. Considering I'm a manly man my list is quite long compared to others, apparently I take after my great grandfather in that respect. He was the pillar of our family right up until he passed. I only remember meeting him once before he passed and then determining to be just like him when I grew up. When I was about 15 my grand-nan, his wife, changed my life forever. "you know," she said, "you remind me of my husband." World shattered and fell apart right then and there, this is NAN !! shes not supposed to be serious. She validated my entire existence in one simple sentence. Even now I shed a tear of pride.

For gaming, Metal Gear Solid 2 has that one particular scene - anyone who has played the game knows what I'm talking about. It doesn't resonate with me anymore, but the first time I saw it, I was definitely moved. Silent Hill 2 is another - the letter at the end of the game is heartbreaking.

For music, definitely "Black" by Pearl Jam. Another really good one is "Waitin' for a Superman" by the Flaming Lips.

As for film, I cry relatively easily (for a guy, anyway), so I've seen a lot of movies that have made me cry. As for the saddest, I'd have to nominate "Up" as a contender - that opening scene...

While maybe not the saddest thing I've ever read, the most recent sad thing to affect me in a story was in Order 66: A Republic Commando Novel, especially...

...at Etain's funeral, when Kad gives her his toy

I was tearing up like you wouldn't believe and I just had to put the book down for awhile. I even knew ahead of time what was going to happen but it was just so sad.

Movie - anything on lifetime, my mom grounded me one day and forced me to watch lifetime from 8 a.m. to 6 p.m. with her, i was so fucking pissed off at how sad that bullshit was, I raged the rest of the night about it.

Game- Heavy Rain, couldn't even complete the damn thing it made me so depressed

Book - I wouldn't dare read a sad book, as I hate reading books with a passion as is, so probably something shitty that was required to read in high school English class.

I don't know if anyone is familiar with the Malazan book of the fallen series written by erikson and esselmont, but I am and I'd have to say when Whiskeyjack died to name but one sad moment in the series, I damn near cried.

BTW its a VERY good series lord of the rings hasn't got shit on it.

Song. I'll have to say several of the incarnations of Last Kiss. Leaves a sorta depressing feeling.

Game. I'll say the end of Persona 3. Amazing story telling and the usual, and in a sense I love the ending for being what it is, but it isn't any less poignant.
