I'd have to go with the first couple levels of Alien vs. Predator 2 from the marine campaign. There is *nothing* to fight that entire time, but what you hear and *don't* see is what gets to you.

Thought the basement level in Episode 1 was scarier than Ravenholm. Badass freaky music, no lights except for burning shit and holes in the ground. Not fun.

It wasn't scary until you were moving away from one group to gain distance, only to hit another that gang bang you in under a second. :S

The entire game of The Suffering. In its time, it was the scariest thing ever, I have not been afraid of anything since then.

I vouch for the Shalebridge Cradle. Playing it alone, in the dark, with headphones didn't exactly detract from the experience.

Although that Orphanage level from Painkiller was rather creepy, too.

Oh, and let's not forget the Haunted Hotel from Bloodlines... yeah.
