Scariest level has to be the hydroponics lab where you first meet the 'midwife' cyborgs but even the vanilla 'grunts' are scary at first. "Your flesh will welcome us..."
First time I encountered Nurse Bloome I nearly shat myself :S

The storage halls in System Shock 2, on level 1, where scary as hell. Especially with the ambient music.

Also, level 7 and 8 of Arx Fatalis, the ratmen's realm was pretty fucked up, as was the old dwarven mine.

I have to say the ambient music and sounds are a big part of what's scary for me. More so if it isn't jump-out scary but... quiet and suspenseful.
Ravenholme was nice, but not actually scary.
Just... zombies 'n shit.

The first Marine level in Aliens Vs Predator, took me ages to get the courage to do that, eventually I god cheated it, so I could do a dry run, then went back through knowing when and where I'd be getting killed from.

Definitely the Ocean View Hotel in Vampire: The Masquerade Bloodlines. Or Apartment 506 (I think), though that's gory horror rather than psychological.

The moment in Bioshock where the go out and all the splicers came to life, scared me pretty good. Also the valkyr trip in Max Payne is not scary, but creepy for sure. Lastly, if you play any game played in the dark, at two in the morning, with the volume turned up will make anything scary/creepy.

However,I think that this is the scariest game ever. Wish I could have the game for my own devices.

Let's see....The Well in Ocarina of Time. A wonderful trick-or-treat bag filled with undesirable grouchy insects, giant spiders, flaming demon heads, shield-gobbling Likelikes and enough dead ends to keep you reaching in for more and more handfuls of pain.

The region at the "bottom" of the world, below Deep Sky in Skies of Arcadia Legends. No light, just a claustrophobic little region filled with endless mud and strange creatures who enjoy snacking on your face...or rather, your ship's...face. Plates. Hull faceplates. Although De Loco's appearance really brightened things up, because he's great at doing that.

Eternal Darkness gets a nod in general, but in particular the level where you take control of Edward inside his mansion and you have to face the Vampire beast. It's not enough to replace half of the demon's blood with lead: once you've done that, it'll retreat downstairs to replenish itself and scamper lovingly back up the basement stairs to wherever it is you've decided to hide. Extra creepy points because the music will shift just slightly when it's getting closer, and you can start to hear the scratching of claws on the floor...

Yeesh.

I also think that the Dark Brotherhood quest in Elder Scrolls IV Oblivion (the one where you go into the Anvil Lighthouse Basement) was scary.

Shadow Temple in OOT. Invisible platforms, lots of undead characters, and the boss really creeped me out.

alot of the places in Silent Hill (PSX) - i really hate that alleyway at the beginning, damn thing still gives me the creeps (not the end, just going down it)
