MYST!
Your wondering this island all by yourself. Two guys screaming at you from books, both of them very creepy. I was just waiting for the moment when I was going to get stabbed in the back or raped. I knew someone was out to get me.

Game moment that was meant to be scary, system shock two. My best friend laughed at my fear of monkeys, then he played it. I have never been so profusely apologized to in my entire life.

Back in the day?
When I was a wee, fledgling gamer?

The darn deku baba's in Ocarina of time kept me from playing the game. Danger music would play, and those jerks would pop up right in front of me and take a big bite out of my face. That was it.

More recently?
I still do not like fighting the Regenerators in RE4. It didn't help that the first one I fought, I blew off its arms and legs, so it wiggled like a worm up towards me, then mauled me to death by attaching to my neck.

I think the scariness of a video game moment comes a lot from the atmosphere, ie music. Then, the more mature you are, the more the situation scares you: for Regenerators, you have a nearly invincible monster moving towards you in a dedicated fashion, and you have precious few seconds to bust out that sniper rifle or get mauled.

EDIT:

How about the Marine campaign in AVP2?
I pissed myself when you had to wander in the dark with a limited flash-light and infinite spawning aliens.

I agree with most of these games. System Shock, F.E.A.R., Doom 3, Prey, S.T.A.L.K.E.R., and Half Life (1/2) were all played on PC with headphones cranked ear bleedingly loud, at night and the lights off and were scary enough. (Not to mention Santitarium and the older creep-out games like 7th guest which I played when I was younger.) Silent Hill, Resident Evil, Onimushia, Condemned, The Suffering, and Clock Tower were played on consoles in the dark, at night, alone and were equally scary.

But Fatal Frame 2 is the only game that ever made me turn on the lights. Just that blaring static growl when a ghost is near you, and the frantic but purposefully slow panning of the camera as you search for the ghost you know could be attacking you any second. And finally the image of your grisly attacker fades into view, the horrifying visage inches away from your lens and howling for your blood as you reflexively smash the shutter button and barely prevent your soul from being shredded by this demonic creature. You catch your breath and glance at the grizzly image left behind on the camera. Take a few steps forward into a strange room and...

I made the mistake of watching the opening of Bioshock alone...
Took me at least 2 weeks to be able to pick it up again.
I get scared real easily :p

Oh and Resident Evil

Resident Evil 2, when I was playing the 2nd story.

There's a point where the Nemesis thing comes at you after solving a puzzle involving a chalk board; you stop it, good, done, solve puzzle.

Then, running down the corridor to get to the next room, BOOM right through the fucking wall at me.

Got outta there something fierce.

Penumbra: Overture was one game that was really a huge mind-fuck. The beginning of the game is really scary.

And yeah STALKER when you were crawling in the undergrounds.

The level in Condemned 2 where you've gotta escape that place with that other dude. But listen when he says his name, then you both see that other dude die, listen to his name. Then after that, the dude who you're escaping with dies. Then you find out it's just Ethan dreaming or something. But, go down to see that guy with the suit when Rosa tells ya too, when you go in the elevator, look at the note on the wall. It's a memorial for the guys that died in Ethan's dream.

survival horrors like Haunting grounds and clock tower, now those are scary.
To get the full effect play alone and turn off your lights, but even if you got friends with you it's still piss scary.
For Haunting grounds all you got is a dog that growls when someone might be coming around,
and with the multiple endings depending hwo you trap or accidently kill your persuers or how well you treat your dog.
Have ending is when you kick the crap otu of the dog so it hates you and you end up raped, pregnant and go insane

I've played through both Condemned and Condemned 2, the only parts that were remotly creepy invovled mannaquins. other than that, the games felt like they were trying so hard to be scary it just wasn't. I've never tried so hard to get drawn into the atmosphere and failed at it. Plus the cutscene animation style, I dunno...it felt cartoony.

I'd have to go with Rule of Rose on the PS2. It's not so much a shocking game as it is a creepy psychological thriller. That makes it way more ominous in my books. There's just something about playing a defenseless little girl in a creepy run down city of lost children world that's every so wrong.

As for the pure terror you get from scary ass things jumping out of nowhere, I'd have to with Obscure 2, 'cuz as much as RE2 did that to me, its camera and lighting was a lot more forgiving than Obscure's, which just freaked me out incessantly.

Condemned: Criminal Origins. There were a lot of scary parts, including the very end sceno when he's standing in the bathroom, but the scariest part was easily in the abandoned school when the supposedly dead guy in the locker grabs you while you're taking a picture. I made the mistake of playing that alone, in my basement, at 1:00 in the morning. Nearly messed my pants.

Condemned was great and I'm really glad I picked it up a couple of months ago out of curiosity. Mostly predictable except for those damned mannequins. And after I made a point of shooting every single one in the store with the tazer because I just knew they would do SOMETHING it still scared the crap out of me. Your first instinct is to run away but they keep following you until your only option is to stay in the room with them or jump into the unknown. A really great and chilling moment.
