When I was playing Thief: the dark project and I leant forward over a corpse which after a few seconds shot straight up.

Not so much horror as unbridled rage, but...

The fake BSOD in Eternal Darkness, The pirated software detected screen in Earthbound, Using too many cheats in Banjo-Kazooie,

If you don't know, a pirated version of earthbound has twice the amount of random encounters and harder difficulty to pad out the game length. When you reach the final boss the game announces that it is aware that it is pirated and forcibly ends the game while deleting all save data in all files and using too many cheats to advance through Gruntilda's lair in Banjo-Kazooie automatically deletes your active file. The game does warn you though so it's your own fault if you called the games bluff and lost.

Its not the scariest part but it comes close:
Some I'm in Condemned 2, the level inside the hotel.
I had just walked into one of the rooms when one of the hobos shot me, so I killed him and his friend and took his gun. Which I quickly discarded in front of the bathroom where I had picked it up because it only had 3 shots left and I didn't have the holster yet.
So I continue walking down the hallway when the first Big ************ you run into appears.
He literally busts through the wall right next to you like the fucking kool-aid man.
He punches me a few times before I figure out whats going on.
I manage to hit him a few times before the lead pipe I was carrying breaks.
He is huge, and he's been pretty slow so far so I figure my chances of running back to the revolver I left behind seemed pretty good, the revolver is about 40 feet from where I am now.
So I take off back down the hallway and into the room with the gun.
Now I'm sprinting at full speed and I figure the best way to slow down and pick up the gun is to simultaneously pick up the gun while I'm sliding to a halt.
Unfortuantly, I just missed my chance to grab the gun and I slide past it.
"No, biggee", I thought. "I'll go back and grab it quick."
WRONG!
Now, I thought I had a good head start on the BigAssMotherFucker because I'm smaller/light than him, so I must be faster.
WRONG!
When I turn around he is RIGHT there and he punches me INTO the bathroom I had just stopped short of.
So, I'm stuck in a tiny, shitty bathroom with the BigAssMotherFucker with no gun, no weapons of any kind.
Lucky for me he didn't have as much health as I thought left. (probably because he was the first in a long line of BigAssMotherFuckers)
What happened next was pretty bamf tho.
After I beat him he was on his knees in one of those "finish him" poses.
So I grab the seat off the toilet and beat his head in.
I pick up the revolver and walk away.
....
Completely true. And I almost shit myself when I turned around and that MoFo was RIGHT there.
Anyway:

So little backstory, I started playing Call of Cthulu: Dark Corners of the Earth since I loved H.P. Lovecraft and want to get into the Tabletop RPG anyways, I got up to the part "Attack of the fishermen" (The part in the hotel.) and basically shat my pants, the tense fear and frantic locking of doors and pushing slowly while screaming "OPEN GODDAMIT I DON'T WANT TO GET KILLED BY FISHERMEN", so anyway this is basically one of my most scary moments right next Silent Hill 2, the whole game.
That same bit in Dark Corners of the Earth. Damn those heavy cabinets!

Actually the scariest part of a game I've played is probably any part with the regeneraters in Resident Evil 4 and Dead Space.

As always with these threads: Thief: Deadly Shadows - The Shalebridge Cradle.

As mediocre as the rest oif the game was, that level is still the epitome of scary for me. It's execution was masterful, and while I think that Penumbra and Amnesia are overall way scarier games, they don't hold a light against the horror that is The Shalebridge Cradle.

Interesting writeup on how it excels from a level design POV: http://gillen.cream.org/thecradle.pdf (PDF Download). Not by me, but worth a read if you've played the level.

The prison in Silent Hill 2. Including the walk down the stairs to get there.

The Dunwich Building in Fallout 3 because when I went in there i had barely any ammo only like 5 stimpacks and I had the GOTY version so the reaver ghouls showed up with like 500 health. I somehow survived after about 5 trys at getting to the bobblehead but it was just sooo scary. Dont get me started about the little mini story that you can discover there then you start getting the hallucinations. Im a wuss arent I

For me it would have to be in Bioshock. Not because it is a scary game really, but just one moment where I was looting a cabinet and I turn around to see a splicer just standing there, an inch behind me staring at me. Wasn't expecting it and it scared the crap out of me.

Another would have to be digging around in Minecraft and out of nowhere you hear the twang of a bow or the hiss of a creeper.

I'm surprised nobody has mentioned the scene in Bioshock where you first get the shotgun. Those who have played the game will now remember this and the utter terror they experienced when this event first happened.

It's had to say. It was probably Amnesia or Minecraft. In terms of atmospheric scaryness about to have a heart attack at any second, Amnesia. In terms of actually having a heart attack, Minecraft. After digging for miles only to have a zombie or creeper drop down infront of me.

I was just too proud of my shotgun to be scared. My first thought on getting the shotgun was Alright then, let the massacre begin!. I moved slowly in a circle and just banged the fuck out of anything that moved.

I am such a badass.

Naturally, I shit myself when I was innocently tailing a Little Sister who seemed to have lost her Big Daddy...and then I just heard a roar and faceplanted the bottom of the stairs.

Amnesia: The Dark Descent.

The part where those monsters pop up round the corner, the door behind you is locked, & they capture you. That was the first time I went face to face with one, As I'd been pretty stealthy for the most part.

Since no one mentioned it yet, I vote for the Ravenholm moments in HL2. My adrenaline was high while in the whole area and i was actually glad when I got out of that shithole.

The Water Horror chase in Amnesia: The Dark Descent. I never thought I could almost-faint from running away from a virtual invisible monster o_O
