The Ocean House Hotel in Vampire: The Masquerade - Bloodlines.

It contains pretty much every haunted house trope imaginable, but in the context of the game, it is genuinely affecting.

Also, the Storage area in Amnesia: The Dark Descent.

Goddamnit.

For me it was (big surprise) in Amnesia: The Dark Descent in the Choir (The room that's filled with red Silent Hill mist).
I started walking around after I picked up my first piece of Macguffin pie in that room and then I walked up to a bridge. I started hearing the sound that happens when a monster spots you (the creepy high pitched one) so I started running to the door and then as I looked to the right the monster appeared right infront of me in all it's mutated glory just in time for it to kill me! Damn I hate that room.

I'm not gonna lie. The biting piano from Big Boo's Haunt in Super Mario 64 is scary. Especially if you play it for the first time and don't expect it.

Not really part of a game but I was sitting in my room with the lights off playing amnesia and my 7 year old nephew decides to sneak behind my chair and let his face slowly hover into view in the blank TV screen to my left. I nearly died a death lol.

Shalebridge Cradle in Thief 3. Just... all of it. Nothing beats picking a door open only to have a freaking twitching zombie with cages on its head and limbs swing it open into your face, and then trying to scramble back while crouched while pressing FLASH BOMB FLASH BOMB OH PLEEEASSSE.

Other parts of the game were scary too, particularly to someone new to the series. That time on the zombie-infested ship... My god, the wheezing! *shudder*
The mission in that widow's mansion, too, because it had such a melancholy atmosphere that I kept *expecting* ghosts or something.

And then I went back to try out Thief 1 and such and it was so much more terrifying, because the zombies are basically unkillable and so much harder to hide from! O_O

Morrowind and Oblivion had plenty of scary moments too, while exploring dark ruins with the soundtrack (and its warning blares of combat music) turned off. The daedra shrines and ugh, the Corprus victims in Morrowind... >_< Another scary moment was when I had a custom track in the soundtrack folder that actually includes sort of crunching footsteps that sound like they're right behind you. Whenever that starts playing in the game is genuinely paranoia-inducing.

Some friends and I were playing Killing Floor on Hard and they both died...should've left the lights on. Then they wouldn't get to hear me scream like a little girl as the Patriarch clotheslined me.

those creepers creep me out like nothing else, i get the biggest scare and chills when i hear that sssssssssssss or i turn around to find one standing right behind me!
Agreed with you on that one. Minecraft is probably the scariest game I've ever played. But then again, I haven't dared to touch Amnesia: The Dark Descent.

RavenHolm was pretty scary.

Gotta give Props to Metroid Prime, though. The first time I wandered into a room full of Chozo Ghosts, I was scared stiff. It take a special kind of frightening to be truly scary to someone wearing ALIEN POWER ARMOR and carrying around a couple hundred missiles.

The scariest moment to memory when the Licker jumped through the double sided mirror in the police station in Resident Evil II. No matter how many times I played the game, it always seemed to scare the crap out of me.

Midwich Elementary and Ravenholm with those damnable poison headcrabs. Nothing else really comes close, though Lisa's meltdown in the climax of SH1 came a close second.
