Gears of War 1, the first time you see the Berserker, or should I say after it kills that one coward. After hearing no conventional weapons kill the thing I said "not going to happen", granted I was like 10 at the time, and I put my controller down, turned of the console and didn't pick it back up for like a month after watching the walkthrough of that part of the level.

I'd have to go with the OP and say the Hotel in Vampire: Masquerade-Bloodlines. That was fucking freaky, especially whenever the guy with the axe showed up...and when you heard something tumbling around in the dryer only to later find a news clipping about what the police had discovered in said dyer. *shudder*

I once fell asleep playing Majora's Mask and woke up to the moon crashing. Almost pissed myself.

The Rogue VI in Mass Effect 2, the first time you encounter its garbled screeching. Not cool. Made me jump alright.

Yea Majora's Mask was pretty disturbing as a kid.
But for me the scariest was in Resident Evil 0, which I was playing through with my cousin. When I was in the creepy torture chamber-esque basement, a gorilla jumped on my back and I had the controller down and was thumbing through the manual for some reason when my cousin says plainly, "Oh my god, there's a monkey on your back."
"Hm?" I reply, not sure what he means.
"There is a monkey on your back." I look at the screen.
"OH MY GOD THERE IS A MONKEY ON MY BACK!"

The original Tomb Raider was just full of suspenseful moments. Remember meeting Mr. T-Rex? While that was a nice shock, it really didn't compare to when you started running into Atlanteans regularly in Egypt, jumping out unexpectedly screaming their heads off....

And then of course there was the Atlantis and the great pyramid. First part of Atlantis, with all those flickering lights and the Atlantean pods you just knew were going to open? God that game made me put it down more than a few times just to keep from freaking out abit.
