The Fatal Frame games, for sure.
Scariest moments... every frickin' minute of those games. In all seriousness though, there are a few, usually the first appearances of certain ghosts. For instance, I shall haunt the dreams of all who have played the first one by simply typing: "My eyes!"
Also, there was one point in the second one where my cousin was playing (we played through them together: less scarring that way), and he decided to randomly look out a window in first person for no reason. Apparently the developers anticipated this completely random action (??), as a little ghost boy popped up suddenly with a music blast. Just a little ghost boy, not bloody or anything, just... surprising. Completely unexpected. It was one of the greatest jump scares I've ever seen (another one being the one locker in Condemned; you know which one I mean).
