Although none of it really it disturbs me anymore, Dawn of War executions change you :p, The Darkness 2 has some really dark stuff. Like holy-heart-eating-spine-grabbing-from-the-ass-alien-chestburster-shit! It has some of the goriest deaths I've ever seen in a game. And I loved every second of it >.>.

Dwarf Fortress by far. It may be from player "creativity" rather than by design but if you think too hard about a lot of things you can do in that game it's just disturbing. The forums themselves are filled with perfectly serious threads about the most disturbing acts.

Merman farms
Kitten clothing industries
Elaborate deathtraps

and Dwarven Daycare - To quote the thread started for that particular gem "Dwarven Daycare, it's like regular daycare only with more dogs and less care"

Whenever another new feature is introduced to the game the same thing always happens someone somewhere in the community will weaponize it in a some horrific way.

I don't think virtual Violence or even the whole Horrorthing in Games really falls under disturbing, or well at least for me it doesn't. I'm more disturbed by stuff that really fucks with your mind like the whole atmosphere in Portal or really weird old glitches in 1st gen 3D Games. But the most disturbing thing I've ever played is a re4ally obscure m,od for the first Half-Life called Half-Quake. It's still being worked and holy fuck is it weird.

Binding of Isac was seriously screwed up from the get go. Fighting enemies inside the womb of your own mother thats trying to kill you? And you fight with your tears. Yeah, tears.

But yes, VtM Bloodlines was pretty screwed too.

Saya no Uta is a fairly disturbing visual novel, I really don't recommend it except to those that can handle heavy gore, extremely violent sexual situations and the like.

most disturbing game?
in direct confrontation Dante's inferno probably takes the price for most disturbing game i own what with the killed babys and other stuff i dont want to mention.
for subtle it would be corpse party some heavy stuff in there.

The only really disturbing game I remember playing recently is Binding of Isaac.

It's just really creepy, the dissonance with really happy smiley faces and blood and gore everywhere, and piss and shit all over the place.

There's only one game I know of that has you fighting walking vaginas that shoot lasers, not to mention the feces and abundant dark humour (Such as the wire coat hanger item that ends up stuck through his head). The Binding of Isaac is one fucked up game.

And things only get more stange and messed up from there. Damn, that game is awesome... The killer7 of this generation, I tells ya.

EDIT: Also No More Heroes. The boss executions are just brutal, and some of the bosses are really messed up (Destroyman, anyone?).

I would have to second all three of these games. Deadly Premonition was a brilliant sleeper title a couple of years ago. Its just very messed up.

And lets not even get into the craziness that No More Heroes is. That was pure comic gold. And the scary part is that you laugh at the humour but its such a gruesome plot.

I thought God of War 3 was the most brutal game I've seen, until I watched Miracle of Sound's Binary Divide and saw how messed up Darkness 2 is.
