If we're talking not-game-over shocks, I can say I was stunned by Braid's ending...
There were multiple endings in Riven? The only bad ending I can think of was getting trapped in the prison book (and I can't remember whether that actually happened)
I remember all the bad endings to Exile though, nothing like a good bludgeoning of the head.
1. Use the trap-book in the rebel hideout: Burn to death.
2. Use the trap-book on Riven itself: Gehn forces his guinea-pig sidekick to use it, knocking you back into the real world where he shoots you in the face and thanks you for revealing the trap.
3. Trap Gehn, then release him: He asks WTF you're doing... but thanks. And he closes the book.
4. Trap Gehn, then release him in the Rebel age: You horrible, horrible, HORRIBLE bastard.
5. Don't trap Gehn, break the fissure: Atrus comes in, is promptly shot in the face, Gehn thanks you and shoots YOU in the face, Riven eats itself.
6. Trap Gehn, but don't release Catherine: Atrus is mortified, but links away as Riven eats itself.
So yeah, it has bad endings.
I didn't think the bad endings to Exile were shocking, seeing how they ended exactly as one would expect when your bad guy is played by Brad "I Will Eat Your Soul" Dourif.

The Riven ones were shocking because people died brutally in all of them, compared to the utter lack of death in the previous game.
