I've only had two companions survive so far - Lydia who I dismissed because I was sick of her whining after the dozenth time I gave her a priceless enchanted item to use, and Barbas who was the best dog ever.

Spiderhorse - My most faithful companion, with me over mountain after mountain. Together we broke every law of physics we could get our hands/hooves on respectively, and fought all kinds of crime from bandits to cave bears. He was killed when a dragon I was fighting got reinforcements. I couldn't save him, but I made damn sure to avenge him.
Spiderhorse II - The identical twin brother of Spiderhorse. Lacked any of his sibling's survival instincts, and was taken out by the first dragon we met, who ambushed us as we left the stable.
Spiderhorse III - Unrelated to the former two, she took up the mantle of equine vigilante sidekick and ATV. She went the same way, but after a bit more time than her predecessor.
That guy from Whiterun who wanted to see Kynareth's underground grove - He died as he lived, punching a dragon in the face.
Melka the Hagraven - Honestly believed I'd let her have her tower back if we killed its occupants. She saved me from a half dozen traps, before I literally stabbed her in the back for 15x damage.
