I've only had two companions survive so far - Lydia who I dismissed because I was sick of her whining after the dozenth time I gave her a priceless enchanted item to use, and Barbas who was the best dog ever.

RIP
Spiderhorse - My most faithful companion, with me over mountain after mountain. Together we broke every law of physics we could get our hands/hooves on respectively, and fought all kinds of crime from bandits to cave bears. He was killed when a dragon I was fighting got reinforcements. I couldn't save him, but I made damn sure to avenge him.

Spiderhorse II - The identical twin brother of Spiderhorse. Lacked any of his sibling's survival instincts, and was taken out by the first dragon we met, who ambushed us as we left the stable.

Spiderhorse III - Unrelated to the former two, she took up the mantle of equine vigilante sidekick and ATV. She went the same way, but after a bit more time than her predecessor.

That guy from Whiterun who wanted to see Kynareth's underground grove - He died as he lived, punching a dragon in the face.

Melka the Hagraven - Honestly believed I'd let her have her tower back if we killed its occupants. She saved me from a half dozen traps, before I literally stabbed her in the back for 15x damage.

RIP My horse, I didn't realise the lake wouldn't cushion the fall. Speaking of, is there any particular hold that sells faster horses?

RIP Dark Elf Mercenary from Drunken Huntsman

I didn't have you at my side for very long. One moment we're fighting Forsworn in the Reach, and the next, you tripped and fell in the river. I watched her float away, down a waterfall. After some tracking her down, she had the slightest bit of health. I was about to heal her, and then a skeever out of nowhere bit her, and she died.

Worst. Merc. Ever.

R.I.P Lydia - Killed by a necromancer. Let it be known that no necromancers in the fort survived that day.

R.I.P Random lady who I beat in a fist-fight in Whiterun. Actually screw it, she was a terrible companion and had no concept of stealth. She had it coming.

Lydia Death by arrow to the back of the head.

Why is it when I'm trying to sneak for crit my companions suddenly decide to go it alone? By running in between me and my target?

But whatever with Death Thrall she's still following me around, at least until she dies again...

How does she die, I ran from a city with guards attacking us and was like "boo hoo she's dead" I get home and sleep then I wake up and she's there. Then we were fighting a dragon and she did that injured animation the unkillables do and I throw a heal in her and her life is like 1%, either I saved her right on time or she can't die.

R.I.P Stray dog.

You randomly decided to follow me, but you activated a trap and got yourself squished against the ceiling. I then went and looted your corpse for anything, but you had nothing because you were just a stray dog.
I dont know why but...This is quite possibly the funniest thing i've read today.

Clegane]R.I.P. Lydia, I'm sorry I ran away and left you fight a Frost Troll on your own...
A frost troll kicked Lydia of the cliffside.

So yeah, MIA Lydia. Hope you landed safely.

R.I.P. Lydia. I sacrificed her to Daedric Prince Boethiah. Good times.

R.I.P. Nvidia 9600 GSO. The dust on your heat sink was too much, making the strain of Skyrim too great. You will be missed replaced.

Lydia died because I Shouted her off a mountain.
Some random Redguard woman from Whiterun died because I sacrificed her to speak to Boethia.
A dog (That turned out to be the immortal companion of a Daedra) died when meeting the end of a rather large axe. Wielded by me.
I also killed some people I befriended and tested some Shouts on.

RIP my horse, eaten by dragon. Not a bad way to go. No companion casualties yet, mainly because I haven't actually let Lydia out of the door yet.

I had a companion before Lydia, actually. Jenassa, the Dark Elf assassin-type lady. I use her as a sort of pack-mule. Also, she uses a bow, so it balances out my character build (which is basically a spell-sword).

RIP Vigilance, died facing down the lackeys of Nepos the Nose.
Except not, because then I reloaded my save and left him just outside the house. ^_^
