Its funny to see some of the things ppl are saying they will do first, forgetting that this is an elder scrolls game and they are going to start out in a worse state than the beggars and can be killed by the first bandit they encounter...
As for the reality, you will start off trying to figure out what does what and where you are supposed to go for at least the first hour. I dont mean to be mean or rude but that is how most of their games actually start on the first play through.

Start inspecting NPCs to see which armor I enjoy the most. I don't care about stats I always go for looks in TES games.

Seriously though, I'll probably just be a wandering bandit slayer (if Bethesda includes Oblivion-style level scaling). If not, I'll probably find out what a Breton-Argonian marriage looks like.

Make my way out of whatever prison I end up in this time around. Seriously, ever since Morrowind Bethesda has had a thing for the world-saving hero being a prisoner.

After that...steal stuff. Join the Thieves' Guild and Dark Brotherhood(if either are in the game). Traditional sneaky-type stuff I do in Elder Scrolls games.

Ever since Morrowind there's been one Elder Scrolls game where the protagonist has started in prison. I wouldn't really call that a thing.

The first thing I'm going to do is test if "kill" still works in Skyrim. Then, I'll run around town for an hour pretending that I'm Kira/God.

Find nearest town. Punch guard. See if I can get out again.

It's always fun to try.

After that I'll probably go spelunking, I do love some of the dungeon designs BethSoft throw out and some of the stories can be captivating or just interesting. Plus good loot. So go into a few dungeons, have a nose around and play with the controls.

Then I'll go looking for a few good mages to see what kinda spells there are, I never made a mage I liked on Oblivion so it'd be nice to make something out of my comfort zone.

Then...probably go for some side quests. I tend to do the story as a last resort in most games.

Something that makes guards shout " Stop Criminal ", " Pay the fine ", " Why won't you die " or a civilian to say " Murder! Someones been murdered! "
God i hope they have as equally funny/ good sayings for the guards this time around, maybe a little more varied but still along the same lines

ima splore! Get a feel for the game and how it works. Probably exp grind a bit and spelunk. Completely ignore the main quest like usual.

I'll take up doing the main quest.

Predictable, yeah, but I want to see if I can feel the drama and adventure of being a chosen hero, and a Dragonborn no less.

Imagine, upon leanring the revelation that you're a Dragonborn and it is your destiny to deal with the Dragons having run rampant in Skyrim, you must embark on a quest to learn hat going on and what you must do to stop it, learning abit more about yourself and the world along the way...

Okay, I can't continue here, this is getting corny, and I'm not sure if that's exactly what Bethesta is aiming to pull off, but I think you get the idea.

But after the Main Quest, I might run around searching for fame and glory, or build up enough money to buy the most fancy-looking house in the game.

Personally, I will probally go hunting. Whenever I roleplay in games like these, I always start out as a hunter, humbly enough. Of course, I will also go out and start stealing stuff. I need to find that thieves guild after all.
the most inappopiate thing possible. wear absolutly nothing, and beat the whole game wearing nothing. than see if argonians still have tails coming out of their butts

If the whole cooking thing works I shall become the world's greatest traveling chef/archmage. Going from town to town killing deer with hilariously overpowered spells, cooking them and selling it to the delight of all.
