I have had too many insane glitches to count. Windhelm seems to be the center of a lot of glitches for me. I have a save right before exiting the city where, after I exit the city and load the overworld, a horse comes flying out of the sky at terminal velocity and bounces way up into the air after a fatal impact before falling into the mote under the bridge. Happens every time I load that save. It's awesome.
Best one I have seen so far is a giant attacking a mammoth (for whatever reason) then doing the typical giant glitch so seeing a mammoth go flying into the skybox was just.....just awesome. Really wish it landed but :S
For me, Markarth was routinely invaded by random enemies. Granted, staying in Cidhna Mine for a week may have deprived me of a bit of context, but someone could have at least warned me that there was a giant playing NPC-golf right in front of the exit.
I once travelled to the College of Winterhold, only for it to be under siege by about 20 draugr and miscellaneous bandits.
Of course, the character hadn't learned stage three of Unrelenting Force, so the opportunity to FUS RO DAH all the fools off the very narrow bridge went to waste.

Dude, SO MUCH THIS. I mean, I'd LIKE to kill Paarthurnax, if the stupid-ass game would let me... but he just sits there calmly, staring while I pump arrows into his face like Legolas into the Haradrim.
Windhelm is the epicentre of many of skyrims's glitches. Most of it is the result of the Mass Murderer quest line being almost completley broken unless its the first quest you do in the game and even then its touch and go.
One of my favorites came when I completed the quest related to Frost (the horse). At the end of the quest I decided to let the guy have him, since I already had Shadowmere from the Dark Brotherhood, so I said "Here's Frost, have a nice day," and he rode off... but then the next time I went to Whiterun, there he was, chilling in front of the stable... Buried halfway in the ground and standing next to 2 copies of himself. I have no idea why they're there.
It's some kind of glitch that happens to some people, basically you're "ambushed" by random enemies when you fast travel somewhere, regardless of whether it's inside a city or not.
Once I fast travelled to War Maiden's in Whiterun and by the gate, there were the 2 guards and 2 Alik'r Warriors hanging around as normal, but also 2 Frost Trolls and a Draugr waiting for me to turn up, just standing there shooting the breeze with guards Alik'r Warriors.
As soon as the cell loaded and everything became relevant to my presence, all hell broke loose. One of the Frost Trolls instantly killed the Draugr with one swipe, while the other was set upon by the 2 Whiterun Guards. The other Frost Troll engaged the Alik'r and heroically, Nazeem charged into battle and started stabbing the distracted Frost Troll in the back with a iron dagger.
Another time with another character, I fast travelled to the stables outside Solitude and there were about 20 random bandits, a troll and some wolves seemingly waiting for me. The first time all hell broke loose as the cell loaded, with the bandits attacking the troll and the wolves, then with a few guards, the stable owner and horse attacking the bandits.
The next time I reloaded the save and as soon as I fast travelled, I cast the most powerful mass calm spell to stop everyone going nuts and give me time to take some screen shots (and to decide how I was going to annihilate such a massive group of passive enemies).

Basically (viewing backwards) it's a few screens of the first time when everyone went nuts immediately and started fighting each other, then a some shots of the "Troll and Bandit Party" after I reloaded and managed to calm everyone before they attacked each other, then some shots of the aftermath of a Shield Charge and AOE attack on a large group of enemies and the clear up operation I had to do afterwards (i.e. make a large pile of bodies and cremate them).
As for what causes this glitch, I have now idea... it's probably just a problem with the Gamebyro that manifests itself once your game lasts for too long or the save file becomes too large (the same thing happened to me in Fallout 3).
It's just one of the many gitches in these games that make them craptastic fun and are probably too amusing to be fixed, instead becoming "features".
I know that feeling, bro. And I've got a glitch that will knock your socks off.
Any time I enter water, the game crashes. On the surface, not a big deal. However, upon closer inspection, it locks out all of the end quests of the factions, because they all involve water in some way.
So, as you aptly put it, they dun goofed.
