I think the "Dazed" debuff in WoW. The whole point of Dazed is to make sure you, the player, stick to the levelling zone Blizzard want you to stay in (you can't run through high-level zones, to get to other zones for your level, without dying umpteen times because of getting Dazed).

Hmm, probably the last level in MGS4, on Big Boss Extreme, going for no alerts. Its impossible, i had to try like probably 55 times, not exagerating, so goddamn annoying. also, beating that game with what you thought you had no kills, but you end up with one random kill from some damn enemy and you cant go back, talk about wasted time.

In kingdom hearts, some of those boss fights, i almost snapped my controller in half. In fact, one of mine broke after i repetitively bashed it against the ground. But that was a long time ago.

I always use 2 or 3 save files. Mine never corrupt anyway, but it always happens to my boyfriend.
Saying that, I can't get past most bosses (ahem, Symour FFX) so that's probably the games way of making up for me never having corrupt files.
You're a lucky one, nothing worse than loading up a game you've been working on and seeing those two words...it's soul crushing. For some reason I have this...OCD thing that I don't like to have more than one save file for a single playthrough. So I overwrite. It's bitten me on the ass tons of times though, trying to get passed it :p

ya expecially when you have achievments where you have to make a choice... if you choose one you cant get the other unless you had saved smartly.

Happened just yesterday actually.

Playing Star Ocean: Till the End of Time working on the Maze of Tribulations.
After fighting the 4 mini-bosses in a row and figuring the final challenge of the zone was near, I decided to backtrack from the dungeon and save in the nearby town.

For those that don't know the Maze is an optional dungeon after the main game with zero save points in it.

Well, it turns out the last boss of the zone shows himself when you try and leave that room...Yeah...losings 4 hours of work in that damned place was fun...

Well me and a friend got a bit bored playing CoD 4 one day, pause for dramatic inhaling, and we decided to play "Harry Potter: and The Philosophers Stone". We got fairly far into it, and had taken it extremely light hearted up until then, when suddenly it jammed (us having not saved it at all) and we just got really annoyed.

One thing that really, really gets on my nerves is getting back stabbed by spies in Team fortress 2, yeah sure, it's their class thing, but when a spy cloaks up behind you and stabs you and two allies in the back....

One thing that annoyed me went as this:

I'm playing CTF on a custom map on TF2 (it's two trains, similar to the Unreal Tournament map, I beleive). They have a sentry nest in their intel room. My medic is at full ubercharge. I run in... he doesn't uber me. (BTW, we were the only two people there, and I'm a demoman).

Then he dies. The reason for not ubering me? He didn't think I could kill them, so he thought it would be easier to just let me die.

Gotta be the 'corrupt file' nothing more heartbreaking than firing up a game you're deep into and thoroughly enjoying only to get the ol' DATA CORRUPTED come up.

The death of Gunpei Yokoi. Everyone praises Miamoto but Yokoi contributed far more to gaming in his tragically short life than Miamoto has and he is still up and kickin

Well, I am pretty much old school (and understand that there really was life before FF VII):

1) The dark elf/dark dragon battle in FFIV
2) Sagat in the arcade version of SFII (I counted one time- every third or fourth time through the computer made him invincible).
3) The fact that there was no need for Summoned Monsters or Black Magic in FF XII- In the time it took you to cast a Fire Spell (or even Ultima for that matter), you could do twice as much damage casting haste and bravery on your fighters and simply slugging the monsters to death (the only purpose for black magic is, of course, during the last boss fight where the boss becomes conveniently immune to all other types of damage).
4) The ending of FF VII- okay, I understand the "Open to interpretation thing", but I felt cheated after spending all that time to beat Sephiroth OWA.

Any game that requires me to do escort missions. Oh the horror.
I agree...it's just simply more fun to find ways to kill them.
Some of my favorites:

1) Combine land mines and claymores against Meryl in MGS (room right before Psycho Mantis)
2) Lobbing grenades at Emma Emmerich in MGS 2 (as she is walking along the piping during the sniper part).
3) Stinger missile at Emma (suprisingly little hangtime there)
4) Sniping Emma (Dude, I'm sorry, that chick was just annoying)
5) Killing all of the Marines in ANY Halo Mission (especially with Plasma Grenades).
6) Shooting down your team members in Star Fox (any version).
