The thing I never get is why can't Steven just try licking it or apply salvia on the broken gems? Ok granted I assume it won't work since his mother wasn't able to heal them?

As for the centipeed, some people has guess her name would be "Chrysoprase" http://www.gemselect.com/gem-info/chrysoprase/chrysoprase-info.php . It sometime to do on the way it acted (it protect Steven) and that gemstone personality thing.

As to what the centipeed former self

Spoiler: She kinda look like Malachite well she has a quatz hair aka Jasper and Amethesyt

Try to watch closely at the part that did resemble her humanoid form.
