Which island? Japan? Great Britain? Iceland? Or one of those tiny, boring, uninhabited ones?

Resident Evil 4. Game is pretty perfect imo.

Although, probably WoW? It seems like that kind of thing would keep you entertained for much longer than any one single player thing would, but i don't know.

WoW. Assuming you have money to pay the subscription-fee and an internet connection.
TF2 if you only have internet connection.
Otherwise... No idea.
EDIT: Got inspired by the post above me. Without internet I'd go for Oblivion.

I'm not sure why people are bringing up odd questions and games. When this question is posed there are a few simple assumptions

1) You're on a deserted island
2) You're alone (though 1 kinda makes this redundant but hey!). It also encompasses the idea that you won't be able to play WoW or TF2 or any other multi-player game.
3) You're going to be there for a long long time
4) Ignore problems of power (maybe you have a solar panel) or reliability (assume you don't dump your game of choice into a fire or into the ocean)

So the idea would be to think of something you'd still want to be playing 5-10 years out.

Thus the question usually encompasses the qualities of re-playability and uniqueness.

If you're old school, you might go with your favorite Rouge/Nethack clone.

Perhaps something like Diablo1/2's random dungeon generators might be appealing as well.

You could probably spend that much time trying to get 100% completion on Just Cause 2 or Far Cry 2.

Though I'm not a fan, I'm sure many would like the GTA/Saints Row/Red Dead Redeption's vast open world.

Finally I'm sure casual gamers could either do Farmville (stand alone) or Wii Sports until the end of the universe.

Multiplayer/subscription/accessories aren't generally clear. If I have access to online play and any ongoing content, plus no worry of system failure then without doubt Rock Band 3. With lesser considerations it goes to Magic Online then WoW, then Starcraft 2, then Neverwinter Nights 2 (if I have mods but no continual content) and lastly Bayonetta.

I'd go with Fallout 3 GOTY and all of the mods available...

Followed by a epic survivalist Beard and tan...

One game and one game only eh? I'd have to choose Star Wars Battlefront 2. I could play that game until the end of time.

What the hell good is a game going to do you without a console to play it on.

Or a television.

Or Electricity.
