My strangest death is probably when I was playing AVP2. I was playing as Predator, I blasted an Alien that was about pounce to me and the acid splashed and killed me.

IL-2. I dropped a high-altitude bomb, then got intercepted. I do a split-S towards the ground and promptly get blown up by my own free-falling bomb.

In Fallout 3: The Pitt, I was in the Steelyard, and started climbing the large tower in it, whereupon the building I was standing on went no-clip and I fell through it to my death.

I was playing Fallout 3. Just innocently minding my own business when a brahmin flys out of the sky to my right, slams into me and kills me instantly. I have never been more scared to go outside than after experiencing that.

I cannot describe this without sounding like I'm investigating some long-forgotten tribe. In the Swat 4 culture, there are ritual suicides commited with the Door Breaching Charges on a regular basis.

In the final battle of Half-Life 2: Episode Two, I was standing right next to a Strider and trying to fire the device at it. Suddenly, I was launched up into the air, even higher than the top of a strider, and couldn't do a thing until I crashed down to the ground and died.

I still don't have any clue what happened.

On halo 3, driving a warthog on forge with a friend, glitch made me jump out of the car, the screen lagged before returning to normal speed, where I was in front of my still moving warthog which hit a barrel, did a side roll and crushed me.
