I'd like to say this happened to me, but my friend was screwing around in one of the random shacks in Crysis, and he opened both doors to the refrigerator in that shack. He closed the top door and... it killed him. We tried to do it again for a few minutes, but as expected, it didn't work.

i was sniping in Halo 3 in blood gulch, something i hardly ever do because i hate to snipe because i hate snipers because they are cheapskates who cant find the melee button and dont know how to one-two with the plasma pistol. some guy on the other team managed to sneak around and drive a warthog all the way over near me pointed at me. he then melee'd it and it splattered me. wtf.
Walking in front of a parked warthog or just... walking in Half Life 2.

Killed through three walls by headshot from FMJ gun (modern warfare 2)
Stuck by random plasma from across the map (Halo 3)

I was flying a helicopter on Battlefield Bad Company 2, and ran into a UAV. The UAV won. Helicopter bursts into flames, I die. The UAV is unharmed.
Destructible environments in games are always hilarious when it comes to random player deaths - physics have never been more fun! In the original Battlefield: Bad Company I was running towards a door when suddenly I flew through it and died. Did I trip? I don't know.
In Bad Company 2 there's a section where you're in a jeep with a grenade launcher mounted on it. A shit-tonne of bad guys chases you for a while, and the only way to take them out is with the launcher. I blew up a quad bike a good 100 metres in front of me and saw it backflip into the sky. Awesome, right? It gets better.
As soon as I reach the point where it exploded, the fucker lands right on my jeep, presumably killing the entire squad. I like to think my pursuers laughed their asses off afterwards.

This isn't entirely relevant, since it wasn't actually me that died, but the strangest and by far death I've seen in a game was the first time I played GTA IV for the 360. I was driving really REALLY slowly, because I was lost, and was driving through a gas station, and as I was doing so, an NPC was getting out of their car, I clipped their door, which slammed into them, and they crumpled to the ground. I've tried a million times to replicate this, but no luck.
I got another fallout 3 one.
I was hunting the Behemoths and apparentley i ran acrosed a random event, want to guess which one? the fucking firelance, i got smacked in the skull by it and i blew up into a billion little bits.
