setting off one of the largest car explosions in fallout 3 with my fat man launcher.

Hit the mutant but caused all of the cars that he was hideing behind to explode, those explosions set off other cars, ect.

I was cooked to perfection.

I've had the occasional grenade accident, the "oh shit I just pressed the wrong button and jumped out of a plane without a parachute" experience, and once when I was playing CS with my buddies and we were only 2vs4 or something like that, I was a bit stressed out and accidentally shot my friend in the face just a second before the terrorists planted the bomb. We still had a chance though, he was and is an awesome CS player and I was still pretty good despite my clumsy accident with the famas.

Actually wasting my time on leveling up Kiwi on Shining Force 2, that was such a waste!
No way! Kiwi is freaking bad ass. His flame breath and ridiculously high defense destroy any non mage character.

For me, it would probably be killing off important characters in Neverwinter Nights, repeatedly.

'X-Com: Enemy Unknown'... Cleared 90% of a crash recovery map and knew I had the lone survivor pinned inside the saucer. So, pulling from my studies of military tactics, I set up a double volley line behind the cover of a short stone wall in front of the ship's single exit.
Code:
...[__-__]

..wwwwwwwwww
...sss  sss
...SSS SSS
w=wall
s=crouched soldier
S=standing soldier

Theory: Reaction fire from my soldiers would ensure the kill before the Grey could get off a shot... Which is exactly what happened, sorta.

Practice: The alien stepped out of the doorway to immediate gunfire from my troops. A few of the crouched men uselessly plunked shots into the side of the protective wall. Then, almost to a man, the standing rear ranks placed a round into the head of their kneeling brethren in front of them. Then either paniced and continued firing at the others, or were calmly plasma'd by the big-headed midge before he ducked back into the ship.

Sadly, my last save was from prior to the detecion and fighter scramble, and for some reason the game decided to forgo allowing me a rematch, the bugger.

I was playing training mission (forgot what game), and i had a tazer. If you used the tazer on an enemy for a long enough time the enemy would ignite in flames. I did this to some one you are suppose to stealth kill than i stood on the body looking down at it.

Little did I know the fire was killing me. I didn't even relize until my guy keeled over wiggiling in flaming pain.

I have also had my fair share of crushing myself with care packages, I have even killed teammates in hardcore because they were taking my resupply care package

Probably things like destroying buildings in Crysis when I was inside them.

And... Crawling up the middle of the street in CoD4 multiplayer on my friend's PS3.

1. Lead an entire team back to the beginning of the "Hard Rain" level in a Vs. game in L4D2 thinking we were winning. Got it on video too...

2. Trying to play World of Warcraft on recommendation of a friend thinking it "sounded kind of fun."

3. Believing that Final Fantasy could still be a good series after Sakaguchi left the production team (read: Final Fantasy VIII)

In Red Orchestra on Staligrad Kessel I dove off the second floor trying to stab someone with my bayonet. I only got him in the arm and died when I hit the ground...

Using the Scroll of Icarian Flight in Morrowind... You know, the scroll that you get off of the wood elf that falls from the sky. I hadn't saved in a an hour or so and I used that scroll and landed on the ground. I died.

Only game i know where you can tazer people like that is Urban Chaos: Riot Response

OT now, i dunno if it's the stupidest thing but definatly most embarrising thing that happened in a game. I was playing CS:S on a gun game server and i got knifed by a bot, i had to hang my head with the shame.
