I'll throw the occasional lighthearted jab at my sister, and sometimes I do wonder whether it's a bit too much. Not that I ever flat-out insult her, but I'm the only one that ever does it, and I think this might be due to me having the condition that I have and her not wanting to say something she feels might kick my self esteem even lower than it already is because of it. This when I know she has her own self esteem problems, which certainly aren't as bad as mine, but they're still there.

At the same time though there's a certain behaviour she'll exhibit around me and my parents, that she wouldn't be caught dead doing around general acquaintances, because she knows we'll accept it without judgement.

My friends and I do it, but we've got a rule that "shut up means shut up."
This was implemented for the exact reason you made this post. We don't want anyone to feel as if they're being attacked and we don't want anyone to be hurt. There are subjects we consider "taboo" (like my friend's dad's death), but generally, it's a free for all.
As for what we do, most jokes are just questioning each other's masculinity/sexuality. I like to take things my friends say out of context too. We refer to each other as "****" quite often, but that's just a part of being Aussie. :D
