A sudden toning down or lack of action generally tips me off. That or the impression that I'm heading toward some kind of arena

Errm music change/ tempo change is a big one. You just know somethings coming when some sinister music comes on or the battle is getting really heated with the small enemies and the tempo goes up.

When you walk through a door and spend a minute walking down a hall that is completely devoid of random encounters, ending with a large door.

How about a combination of the 'arena' and large/interesting/'inert'/statue/blob thingy standing in the middle looking all 'innocent' and 'harmless'. Then your character is like: Oooh! Whats this? Steps forwards and BAM! All exits blocked. When will Link ever learn?

The Spoon feeding of ammo/health is defo my fav, you can be playing a Co-op for the first time with a friend, and come across a room full of ammo and the like, in front of a door your meant to go through. It's almost instinct when you turn to each other and say "boss".

Suddenly, you can knock all sorts of shit over or interact with all sorts of buttons and there's all sorts of traps in the room.

Bigger, opener area (assuming that hasn't been the whole game thusfar).

DRAMATIC MUSIC.

Cutscene of your character walking into a room, almost always from the front, and doing one of the following:
1. Pointing their gun at something that animates.
2. Putting their gun away and staring in awe at something and then something else animates.
3. The door slams shut and they spin around in shock to face it as behind them something animates and then they turn back around and draw their guns.
4. They walk up to pick up their lovely artifact of doom or whatever from the middle or back of the conveniently large room with relatively few pillars.

A character "gets a bad feeling about this".

If the screen gets dark in a 2D, you know you're in trouble...

Game suddenly give you a whole new awesome weapon? Again, watch out, in some games that's a red flag.

Watch how long it's been since the last boss. If you haven't fought any for a while, chances are you're about to meet something.

Are you about to leave an area you've been travelling through? Hope you've still got lots of potions on you...

Have the enemies been getting larger in number? In some RPG's the worst enemies are before a boss. If you're struggling to stay alive in the face of these enemies, its time to stop advancing and do some training.

Thats all I can think of.

A shopkeeper in the middle of nowhere, its like "Hey, do you wanna buy some of the crap the people who couldnt get past here dropped when they died?"

Or

A cutscene when you walk into a new area.

Or

Or when it tells you that you are about to fight a boss... dont know why, but I always get the feeling a boss battle is about to start with this one in particular(!) XD

You enter a room and the door immediately locks itself (Legend of Zelda)

or

you look on your map and see you are standing in front of the last unexplored room in the dungeon (Again, Legend of Zelda)

or

the door you have to open has a big, fancy lock on it. Bigger and fancier than any other lock in the dungeon. (Do you really have to ask?)

Just a giant door is a pretty good sign of a boss fight. Also if said giant door is in a large and mainly empty room with possibly a few supplies.
