My vote goes to the "closet scare" in the 1st CONDEMNED game for 360.
Ninja'd by the OP. That game was super scary.

As for the the biggest shock, well I think it's still got to be the big reveal in the original Knights of the Old Republic. Maybe it was because I was still young, but I really did not see that coming!

The ending of Braid.

Also, the Blue Screen of Death on Eternal Darkness: Sanity's requiem. I was so shocked by it I sent the console to repairment. The funny thing is that they sent me a new GameCube XD

Ever play STALKER: Shadows of Chernobyl? Well, true to the name, some of the creatures stalk you to an extent.

I realized the truth behind the developers promises deep within a sewer system. After tearing through a group of mercs/looters, I came to loath the quiet. All that could be heard was the dripping of distant leaks, the scuttling of tiny creatures. Every now and then, I'd hear footsteps. Every now and then, I'd open the dark with my shoddy flashlight. I'd find nothing.

Eventually, I came upon a well-lit room. I could see pretty much everything, and I was slightly relieved.

Footsteps.

Wary, I stepped into the room and perched in the corner, looking back from whence I came.

Footsteps.

I stared into the shadow and found two white motes of light glittering in the darkness, staring back. Curious, and startled, I opened fire. After emptying half a clip, I found the bullets had been wasted on the ground.

I crept through the green glow of the room, watching my surroundings with blood-shot eyes. I heard the pitter-patter of footsteps once more, and spun around. Face-to-face I saw those glittering eyes, held by a grotesque face. An unholy beast with tendrils for a mouth, wailed in anger, and lashed out at me.

Much running-and-gunning later, I stood victorious over the beasts body. Bloodied and beaten, I moved on.
Oh I know, when after you help another bunch of STALKERS fend of the Russian/Ukranian military, then you go into the sewer system, yeah, that part gets me all the time, I know its coming but it still scares the crap outa me.

But scariest moment for me is AvP(99) Marine campaign, you kill your first alien then boom, yoiur tracker goes of the scale, scared me so much I closed the game and aye played it since.

Fatal Frame 2 is the winner for both shock and scare

the shock came when i decided to go into 1st person mode to look out a window and get a lay of the land and the moment i do a friggin ghost kid pops up right if front of me!

the scare was when i had to fight the ghost of the woman who fell down the stairs. Everytime she reapears she screams as she falls and passes by literally inches from your face. I found myself dreading to go back in the that room, which sucked because you had to pass through it several times during all the back and forth exploring.

Batman Ark ham Asylum. The bit where it makes you think your disc has stuffed up.
Don't remember that bit, when did that happen?

It happens when you are captured by scarecrow and you play as Joker for a little bit while batman is tied up, the game 'glitches out'.

And I agree that's one of the only moments in game that really had an effect on me even it wasn't a shock or scare, just a "Wtf is going on??" moment. I even tabbed out and googled thinking it was a bug lol

I was running down a hallway in RE3 and Nemesis crashes through the window wielding his signature rocket launcher.

I've played a fair few horror games, but in Fallout 3, I was creeping through this warehouse, and then a Goul runs up and attacks me from behind. I yelped, and then looked around, embarrased.

I'm surprised at how many Bioshock comments there are, those bits are awesome, but not scary.

Happily strolling through the joyous of joy places; Gerudo Desert, in Twilight Princess, I come accross a digging spot. This looks fun!

A few seconds later, I'm immersed in compete darkness, with the faint skulltula sound echoing through my T.V. And then, freaking 12, fully grown spiders surround me. To say I'm afraid of spiders is an understatement; I couldn't play OOT due to the Skulltulas in the great tree thing (the starting dungeon ._.) To say I keeled over in fear would also be an understatement.

My biggest shock was leading upto the the final colossus in Shadow of the Colossus. I won't spoil anything for any poor sod who hasn't played it, but everyone who has knows what happens on that last bridge.

Biggest scare has to go to Resident Evil 2 I think it was. I wasn't even playing (I was about 10 at the time) my brother was. anyway he went into this tight little room and the door locked behind him and he could hear scuttling. He went into first person view only to reveal two giant (like, large dog sized) tarantulas which dropped down from the roof and proceeded to inch towards him. He used his shotty to kill one of them only to have its abdomen split open and have thousands of regular sized tarantulas burst out and slowly crawl over him and eat the character alive.

Arachnophobia explained.

I always know it's coming, and even the first time I played the game I'd long ago figured out what was coming up, but still this one moment in System Shock 2 simply continues to blow my mind.

SHODAN remains my all time favorite villain. Absolutely genius!

I'm not the only one who thinks so either.

I don't think I can select one particular moment in Call of Cthulhu: Dark Corners of the Earth, so I'll just recommend the whole game if you want a fright. Especially the first parts, when you're unarmed. Not that weapons are preferable to sneaking on a lot of occasions later on, but a Tommy Gun feels comforting.

Also, in Vampire: the Masquerade - Bloodlines, there's a part where you explore a haunted house. The biggest shock comes when there's been a lot of furniture flying about, but then suddenly becomes very quiet. A door opens, and a child's toy rolls out and stops. It does absolutely nothing, but the quiet jingle of a bell when it rolls out is such a contrast to the stark silence that it's a lot scarier than anything else the poltergeist throws after you (literally and figuratively).

thank you sketch_zeppelin for bringing up fatal frame series. hint go in early in the game and unlock a few doors then you dont have to cross it so much

but shock has a few tied
Fatal frame 1: closet body
Fatal frame 1: first appearance of broken neck
Clock Tower 3: the opera house where sledghammer just jumps out of nowhere.
(just slightly edged out fallen woman from Fatal Frame 2)

Scare
fatal frame 1: two words, FAMILY MASTER!

ok so you just solve a puzzle you think a keys gonna get revealed.... then family master enters the picture. you go through a boss fight from hell. (all you whove played it know for yourselves how dangerous he can be)

then you spend the whole next night hunting priests and building up to another fight with him, and just in case you thought you had it down....

Quote from me: "Holy sh*t! new attacks!"

yep.... dont get me wrong i still love to fight him even now when i can beat the game in under 6 hours he still gives me the most trouble

Honorable mention go to:

fatal frame 1: 3rd night.... bloody wandering monks! these things are hard to beat (not as hard as family master buuuut) I wont even take them on most of the time unless i need them for the spirit list. as soon as i hear them i book it

Kuon: first boss/ atmosphere.

maybe alot of you havent heard of this? i picked it up in a bin for $9
at first i thought the game was crap with a little questionable voice acting very early on and the chars not moving their mouths while talking.

but then the atmosphere gets too you and your hooked after that fight i needed to take a walk, well woth the $9

When my friends and I were playing HL2 and we were in the bit were you first get the SMG and my friends that was playing turned right into a enclosed passage and we passed a corpse that we passed off as a victim of Man-Hacks and found a box, which we thought was a supply crate, which he then shot and then "OMG, A zombie torso!" We were slightly scared and kerfuddeled, was it supposed to be there? It was mind-racking.

The Hunter in Dead Space was the most consistent scare for me. An impossibly powerful enemy with a one hit kill, who can appear at any time, any where and cannot be killed, merely slowed down? Yeah, the chapters after he's introduced and before you eventually kill him is just pure anticipated horror.

And then there's that chapter where he's following you, and you reach a part of the ship with no air...

Definitely the locker guy in Condemned... For those who haven't played it, you go around opening cupboards and lockers and searching for ammo etc (you do this throughout the game so you get used to it, and it's always safe) then at one point you open a locker and there's a fucking guy hiding inside it who then screams and jumps out at you. Completely didn't expect it, I shat bricks :D

I haven't played it for a while actually and I don't think my recognition is very accurate, but there's definitely a guy in a locker who attacks you at some point. lol

Lets See... The first time I ever played through FEAR I would always have to play it later at night, in the dark with my surround sound headphones on. A few of the sequences where you have to shoot the fast moving ghosts in the pitch black with no flash light got me pretty good.

Also when you would hear Alma's footsteps in a dark empty room behind you definitely made me pause the game and look behind me a few times.
