Ninja Gaiden 2 hands down. Stop the fucking explosive darts. And the bosses...especially the last one (some bosses were piss easy, strangely), some I have tried for 60+ times. I've given up on the game.

rick dangerous. traps everywhere that you couldn't pass unless you knew where they were (and even if you did, chances were it was hard as hell to pass it), each level was extremely long, and you only had 5 lives, no continues.

Ugh. Anyone who has played N+ (NDS or Xbox360) knows that the final Episodes in the game are downright murderous.
Seriously, there is no other game were pressing the D-Pad can be concidered as an artform.
Well, that and Battletoads...

Jak 2, brilliant game. It was hard, painfully hard. Maybe because I was young at the time, but it was very hard and very long...I eventually got cheats for it. Oh god the first Barron battle *shivers*

Dynasty Tactics 2.

Not because of the difficulty in beating the enemies, but for the sheer difficulty in actually getting to grips with how to properly plan and strategize.

Equinox on the SNES. It had the fiddliest precision jumping I've ever seen and had hidden walls, one hit kills, basically everything which makes the game swearingly difficult. I never finished it, I got way to stuck in one of the dungeons (and it was to frustrating to go on).

Either Battletoads on the NES, or I Wanna Be The Guy. I'm not sure if the latter counts because it's kind of meant to be impossible. It doesn't have any pretensions of being an even remotely serious game.

Shit was easy, jump through one of those flames, burn the rope holding the chandalier, and it should fall on the "thing" and kill it. Was I just trolled?

Battletoads is the single hardest game i have ever played. I couldn't get past the second damn level (i was only a young 'un, at the tender age of nine at the time, though.)

The first few that come to my mind are the Ninja Gaiden games and Ikaruga. On a slightly less obvious choice though, I found GTAIV to be really hard, strangely so for what is in theory the king of the mainstream games. It'd take me like 5 attempts to beat every mission, even at the start of the game. This is why I still haven't beaten it.

Devil May Cry 4, with keyboard and mouse. Well perhaps that is a bit personal, but I never play games like that. I suck at button bashing, let alone making intricate combo's. I stopped when I encountered some kind of bat-women (or was that DMC3?). Never again.

A Tales of game on Mania or what ever they call the hrdest diffculty in each game,it's insane. Also the Orginal Ninja gaiden, Bloody Malth and the stage six bosses were just mean. If you didn't have the Whirlwind Slash. Xbox Ninja Gaiden was preety bad as well except the final boss that was kinda of a wussy. SMB three is still a living hell to me

I don't think I can name an absolute hardest game I've ever played, but I can name a few pretty unforgiving ones.

Quiz & Dragons (Arcade) - A fantasy-themed quiz game made by Capcom in the early 90s. Only impossibly difficult if you're completely ignorant of pop culture or history from the 50s to the 80s... a big quarter muncher too.

Dungeons & Dragons: Tower of Doom & Shadow Over Mystara (Arcade): Two D&D games licensed out for development by Capcom in the mid-90s. For a side-scroller beat'em up, it was especially cruel. They actually required some RPG-like strategy between you and the other players.

Gauntlet (Arcade) - BECAUSE THERE'S NO END TO IT!

Dungeons & Dragons: Warriors of the Eternal Sun (Sega Genesis) - real easy to die within only one enemy encounter, even if you did roll your party members' stats really well. There's also no direct narrative given at first for where to actually go.

Phantasy Star II (Sega Genesis) - A great, groundbreaking RPG of it's time, but incredibly difficult if you don't take the time to grind up levels. It's still hard even then for random overpowered encounters and ludicrous dungeon layouts.

Splatterhouse 2 (Sega Genesis) - This game is murderous even on the lowest difficulty. Too many cheap ways to die, no real life bar, not enough extra lives. I couldn't beat this without cheat codes.

Zaxxon Motherbase 2000 (Sega 32X) - A rare 32X title that I actually own a copy of. Makes the original look like a walk in the park. Basically, Zaxxon in low-quality 3D graphics where all the enemies apparently got the memo about faster rocket propulsion for their ships and you didn't. One could complain about the depth perception of the isometric view but the original was the same way, you just have to get used to it.

And last but not least...

Super Mario Bros. 2 JPN (Famicom) - The original Japanese Mario 2 seems to me like the perfect example of what happens if you complain to Shigeru Miyamoto that one of his games was too easy. This game is the epitome of the "Dick Move," there are none higher. The fact that there are Speed Runs done of this game completed between 6 and 8 minutes makes me want to find these people and hold their heads underwater until the bubbles stop.

Those are all I can think of for now... I didn't want to list any that were hard just because of bad game design or glitches, that's too easy of an excuse. There's usually ways to dance around issues like that when they come up during gameplay.
