Weapon mechanics being unrealistic, and scale(I'm looking at you, Halo: Reach).
It's futuristic sci-fi action. It's not SUPPOSED to be realistic.

Anyway, it annoys me when you have to go through unskippable tutorials every time you start a new game.
Yeah, I know, but I human structures should have normal hallways, not 4 meter floor to ceiling.

- Lip syncing is my number one peeve. I'm looking at YOU Alan Wake... When the models look good, the voice acting is good but the facial animations are rubbish it just makes everyone look retarded and bang, there goes my sense of immersion. :(

- The game designers making up stupid reasons (or none at all) for starting a level with no weapons. I'm looking at you AGAIN Alan Wake!

"Oh, I just got in a helicopter with a shotgun, a revolver, a pocketful of flares, several flashbang grenades and a big arse lantern torch. But oops I just fell out of the helicopter, now I have lost ALL my weapons and only have a standard sized torch!"

WTF!? Where were you keeping all those weapons anyway? How did the torch magically SHRINK in that process? Couldn't you still maybe have like a flare in your pocket or something? AHHH! RAGE!

- GTA4 was ruined for me when I found that I could not board the tube train AND walk about it while it moved. I'm sure this is some sort of technical limitation of the game engine but I wanted to go on a shooting spree aboard the train damnit! I was so excited when RDR let me do it instead!

- Sound levels are a major problem for me too and games without the option to change at least the sound effect, dialogue and music volumes should all die. I generally end up putting dialogue up highest, then the sound effects and the music close to that. I find a lot of detail is drowned out through excessively loud music.

When I'm trying to play and people walk in and try to fucking talk to me...
Oh yeah... this! Which also leads me onto, cutscenes which can't be paused. Although most older games can't and a lot of new games can, it still annoys me if I can't pause when I like. Maybe I'm just spoilt :p

My number 1 annoyance is unskippable cutscenes especially after a loading screen not that I'd always skip them but Id at least like to have that option. I just wish I could get the opportunity to just jump right into the game without being forced to watch stuff that probably isnt all that good. The games that use cutscenes well I wouldn't be able to tell if there was this option or not (Valkyria Chronicles)

Invisible walls and bugs are such an immersion breaker. A game could get me all sucked into it's world and caring about hte characters then my character falls through the world, the AI starts driving in circles, or I run into an invisible barrier and then Im suddenly reminded that Im playing a game and games have limits etc

Also, on company of Heroes Opposing Fronts, they call the British guys in maroon berets who parachute and glider into battle commandos, even though they are blatantly from the Parachute regiment, and even have para cap badges and Pegasus patches! How could they?!
Aye, I've heard about that. What makes it harder to understand how they fucked up, is that the para cap badge and people wearing para berets are plastered all over the case!

Most everything stated so far would get too me. My personal annoyance would be drastic changes in the difficulty curve. If the game starts off as ball busting hard but ends like you are playing it on easy mode, that is what really gets to me and that works both ways.
