The first thing that came to mind was the Rainbow Cruise in Super Mario 64. I played that game when I was five and I found that level to be all but impossible. Funny, because I went back and played that game recently and had no trouble. But for all of those child-hours that level consumed, I'll go with that.

Alternatively, Dark Souls. The whole thing.

Everyone's going for crazy hard or frustrating, but for just plain annoying, I'll have to go with Super Paper Mario and that stupid level where you have to get the 100000 coins, and you have to keep hitting the coin box until you get the privilege of holding right and sitting there waiting for nintendo to stop being idiots. Also that other level where there's a fighting tournament and the world blows up so you have to walk through 20 rooms of literal nothingness to see what's up. It's a good game, but there are so many moments where I was wondering what Nintendo was thinking.

Also the monastery in Uncharted 2, where there's nothing to do but slog through a few hundred enemies and there's sure to be a guy with a shotgun hiding behind a pillar every time you think you've cleared a room

I don't know, I found Rayman Origin's water levels pretty fun.

Although that could just be because of the music. :p

OT: Can't say I remember games from when I was younger that had really annoying levels. Maybe I'm just suppressing the memories associated with them. :(

A most recent example would probably be a lot of the missions in Jak 2 from the Jak and Daxter HD collection. This game has a terrible checkpoint system in my opinion. Especially when it came to some of the escort missions. The one where...

You had to protect Krew's men to get the Heart of Mar for example. There were three guys to had to protect and they can barely defend themselves from the onslaught of metal heads you face throughout the level. On top of that, they are extremely annoying. Especially Jynx. Fuck Jynx and his voice actor. Seriously.

The level itself was pretty lengthy as well. And if you or any of the men die at all at any point in the level, you have to start all over again. The fact that you only had four hits with no way to heal yourself doesn't help matters much either.

Hydrocity Zone from Sonic 3 and Donkey Kong Country water levels are good.

Those are the exception though... and you spend most of Hydrocity Zone out of the water (unless you're bad).
A few of the Donkey Kong Country water levels were a ***** but the first two or three were fine. I love the way they look though and, the music is really soothing so they have that going for them.

Stage 4-1 in the original Super Mario Brothers. Hell, even in the more recent Super Mario Cross-over games: stage 4-1 is a freaking *****. Any stage with Lakitu is going to be more of a challenge but something about that one always gets me for at least two lives when I try going through the bastard.

The Flood level on Halo 3 (it was the second to last level). Also the entire Noveria mission from the first Mass Effect can go to hell.

For the person who had trouble at old oney That wasn't too bad you just need to take each death claw on 1 at a time with a sniper then finist them off with a glating gun or glating laser for when you go underneath using bottle cap mines, dart gun and a good shotgun usally works. I love the armor thats underground though. Also bring followers like Fawks.

You probably wouldn't be saying that had you made it to Lost Izalith; A colossal lava pit with a few branch-like walkways, inhabited by a dozen or so 50-foot tall zombie dragon legs who are pretty damn quick on their feet, and who will stomp you to death in a matter of seconds.

Sen's Fortress at least had places you could hide behind for cover and enemies you could guard against.

Edit: For those without the patience to watch three videos, here's the wave layout: (All numbers are approximate.)
-400 skeleton horses that throw cannonballs and lunge
-150 bull-like enemies that can send you crowd-surfing - except the crowd has touch of death
-150 big red rocket launching guys and 250 smaller blue laser-launching guys
-1000 enemies, a mix of the previous four types
-Final boss appears, chases you to the Pyramid through all four of those enemy types
-Keep shooting horses while dodging boss's attacks until the Pyramid opens
-Go to the top where there's an alien ship, fight the boss for real.

I was Assaulting the Necrons capital on DOW Dark Crusade yesterday, That is HARD!

First 5 minutes and your swarmed with Flayed ones, Tomb Sypders and those FUCKING PARIAHS!!!

I gave up so I could go and get more Champions and try again later...

But in all seriousness (No pun intended) I can't do that part on Hard and can only just manage it on Normal..... That one time. And no other times....

All escort missions ever made in every game basically. :p If I had to say one off the top of my head, I'd go with Oblivion's final storyline quest when you escort Martin through the imperial city. If you waited before playing the main storyline until you were a high level, then this mission could be a real *****.
I've probably forgotten worse ones, maybe in the GTA series or something, not sure, I tend to forget the God awful moments in my gaming career.

Dark Souls. No internet. Low soul level. Non-upgraded winged spear. Bellfry Gargoyles. RAAAAAAAAAAAAAAAAAAAGE.

I only managed to beat them by grinding souls from the pathetic beginning-of-the-game baddies until my soul level was around the level it should be by the time you take on Seath the Scaleless. It's purely incompetence on my part, since I was in the same position in Demon's Souls and did the exact same thing. To be fair, once I did beat them everything else was incredibly easy.

STALKER: Clear Sky
Any part of the game with bushes (game doesn't have traditional levels).

Nice that they make foliage in game. Not so nice that AI can see right through it.

The Tower of Eternal Flames from ADOM. There can be no other answer.

Constant health drain, not to mention constant equipment damage/destruction every single turn throughout all four levels of the Tower. To cap that all off when you reach the top of the tower half dead from the heat - the armour melted off your back, your scrolls of teleport nothing but ash and your quiver of 32 arrows of dragon-slaying now only so much charcoal - you have to fight the Ancient Chaos Wyrm and his retinue of fire demons.

And if a cherry were needed for the top of this giant 'screw you' cake, ADOM is a Roguelike which means the game only saves your progress when you close the program and files are self-deleting upon loading - so if you make one mistake it's back to square one with a brand new level one, zero exp pleb.
