Pretty sure the official "most annoying level in gaming history" was the frickin' hover-bike level in Battle Toads. I can only imagine that the number of people who never beat that game specifically because they couldn't pass that level is up in the millions.
I am close friends with someone who has completed that game, on a keyboard. I revere him as a deity of gaming.

OT: Anything that involves water. And that one Mass Effect level were you realise that auto-saving in that game is BS. I'm still considering whether Liara was worth it...

thankfully my brain has blocked out those memories as to avoid re-raging over them. HOWEVER,

any timed escort mission on a water level...

fuck you. i will crush that game into a million bits and eat it before i do a mission like that.

Pit of... fire? Yeah, I buy that.

Super Mario 3: World 7: Plant World

By far the most difficult and frustrating Mario experience. I adore the game, but World 7 makes Bowser's World 8 Lava world look like child's play.

Never mind i'll just go back to getting my ass kicked in UMVC3 instead... Seriously fuck that level. It's not even hard it just takes too long and is obvious filler in a game that's in no need of filler of any kind. The free stat boosts were the only saving grace of that entire section.

Also Sen's Fortress in Dark Souls. It is, without exception, the most trollishly designed dungeon ever made. the very first room has swinging pendulum traps over a narrow bridge with an enemy waiting in the middle of all the traps and fucking wizards firing spells at you. When (not if, when) you fall off you land next to two asshole demons in mud that slows you down, if you make it to the escape ladder there's an item that can only be reached by failing the previously described bridge section.

Fuck you game. Fuck you.

In fact that section is so frustrating I just want to do this in anger.

Wrong on the DS one. There's an elevated ledge near the end of that bridge. Also, there's a spell that reduces fall damage to help you get down there. There's also a ladder that takes you back to the start of the bridge. Just watch out for the 4-5 titanite deamons down there ... and the sludge. Bring a rusted iron ring.

Feros as a infiltrator focusing on sniper rifles. I was screaming at the TV.
You poor, poor man/woman. I almost had to suffer through that, but I also chose to level up assault rifles almost equally to sniper rifles. Those Creepers are a pain in the butt, especially on the higher difficulties where they soak up stupid amounts of damage and can kill you team pretty quick.

OT: The level in Mirror's Edge near the end of the game where you have to destroy the mainframes with all of those guards shooting at you. Actually any level in that game when it forced you to fight.

Dynasty Warriors 4 - The Nanman Campaign
Agreed. By the time i finished it both times all my team including myself had 0 morale. Their morale which increases all the time and the random Meng Huo duel make that level more hell than killing Lu Bu.
