Why has no one mentioned Slippy Toad? Or Otus from Dead Rising? Neither of them ever shut up, they never have any useful information to impart regardless of how much they talk, and neither of them is of any use whatsoever in combat. They even have the gall to complain at you for shooting them out of the sky/hanging up on them. Granted Otus might be more annoying. Slippy may be grating at best but at least you can shut him up for a while by shooting him a lot. Otus is unkillable until you enter a certain mode.

I HATE Dunkoro with burning passion, the guy can't heal to save his life, or mine! seriously Tahlkora heals like a boss, and usually takes up the slack for Dunkoro. I don't know what it is but if I'm not manually making him heal those who need to be healed and revived, he wont do it 60% of the time He just continues attack the as the rest of the party slowly drops like flies around him.

Azmodan in Diablo 3 is pretty annoying. Always popped up, when the only thing I wanted to do was go to the next level. His voice lines were some of the worst in video games I've witnessed yet, as well, horrible writing (or intellect design, if they gave him that silly, stupid and repeating characteristics on purpose)

When he isn't being completely useless, he's moaning about his training, shamelessly brown-nosing his tutors, or trying to get the player killed. His insipid dialogue alone will make you want to shove your lightsaber right up his ass. Luckily,

I completely forgot about him. He's pretty high on the list.

I'm going to go with the chopper guy in Just Cause 2. That voice is so irritating, and the fact that you have to call him back multiple times to order multiple things just makes it worse.

An honorable mention goes to Savoy (the nation) from Empire: Total War. I don't know why, but every single campaign I've gone through, Savoy randomly attacks a couple of weak border towns, then tries to sue for peace for the next 10 turns until their shitty country is completely obliterated.
