Sarevok in Baldur's Gate 2. In the first game, he's the main villain.

Oh yeah, there's also the protagonist of Drakengard 3.

I'd mention the Overlord from the titular games. Your advisor and minions can and do act like pint-sized douchebags, but you're always given the option to stay your hand and to go for a mostly heroic approach to being the kingdom's Sauron expy. Heck, you can even marry a damsel who's into flowers and decorative draperies, thereby turning your castle into something that has more in common with a Medieval Romance airport paperback's cover than with Barad-Dûr.

Does Pyramid Head count? 'cause they establish him as an angel of judgement in the later games. And to be fair, the people he judges, ie the protagonists, are completely assholes.

Kingseeker Frampt from Dark Souls

Heinrich Cornelius Agrippa from Amnesia: The Dark Descent

Sarevok? You can recruit him but he is definitely not a a good guy.
If you lead as an example, he'll turn Chaotic Good.

In Metro 2033 and Metro Last Light you have

The Dark Ones. Things that look like demons that in reality just want peace and have a hard time communicating with humans.

Eh, I guess but I think "genuinely good-hearted and compassionate" is a stretch and it take a long time to get to that point.

Skitter, i.e. Taylor Hebert, from the web serial Worm.

Warning: Spoilers

...

She spends most of the first half being an accidental anti-villain and the rest desperately trying to kill god.

Don't worry, he's evil.

My memory is a little bit fuzzy, too, but I seem to recall him being the most heroic person on that show. So...yeah. Counts.

First thing to pop into my head was Igor from Persona. That huge nose, those bulging, bloodshot eyes and those multi-pointed ears(yeah, in some of his pics, they come to two points). Not to mention his voice or the bone phone. After all that, he's the one creature(guess that's the term for him, entity?) that helps his protagonists and asks nothing in return(save yen for summoning old personas, but that's reasonable).

Seichiro Kitano, the main character of Angel Legend (sometimes called Angel Denetsu, which is basically the title just not fully translated)

He's a kind, gentle natured and pacifistic student who is probably the most genuinely good person you'll encounter in a manga.

Needless to say, the running plot is everyone he meets thinking he's a vicious thug liable to murder them.

First thing that comes to mind is Mantorok from Eternal Darkness. Despite being the "Corpse God" and the "God of Chaos", he keeps the far worse ancients in check and is interested in keeping humanity alive, even if he is manipulative about it. He also looks like this:

And how. He may have started out as a Predacon, but his time with the Maximals changed him for the better. When it came time to say goodbye, he had become no less than a savior:

Hell, even the "evil clone" version of him did the right thing at the end.

Celty Sturlson (from Durarara) who is a modern headless horseman with a big ass scythe and shadow powers. Is actually one of the most moral and sane members of the cast.

Sakaki Shio from History's Strongest Disciple Keniichi. With that build and those scars at best you'd think him an grizzled anti-hero. He is more of a lovable goofball and is very firmly in the "good guy" camp.
