In Halo: Reach I fired a grenade launcher onto our flag and held onto the EMP thingy. I turned around to spy on the enemy and accidently let go. I got three kills.

The funniest death I've ever seen would have to be the death of the blonde woman at the beginning of Run Like Hell (on the PS2), I honestly can't remember her name.
In short:
'Oh no! A giant monster! I won't run but I will get grabbed, picked up, have my head bitten off and spat out with the force of a cannon ball into a computer screen'.
I get the feeling that it was trying to be suprising or scary but it just made me laugh my arse off.

my most hilarious death has to be just cause 2, i called in a plane, started it up, hit a truck and started grinding down a hill upside-down, hit a tree, blew up (but stayed alive) went flying through the window, went straight off the edge of a mountain, hit the side of a cliff and fell into the rotar of a random helicopter. and this happened yesterday strangely enough ¬.¬
fallout 3 i was duing a big pile of mines (like 40 or so mines) at a high explosives skill to kill off a very hard beheamoth in evergreen mills and like the third to last one went went off for some reason it was like a couple of mininukes going off under my feet i flew over the rim of the crater and i have forgotten how far i went it was a long wway though
Just Cause 2: i tethered to a military plane during takeoff and it did a spinny thing and threw me off, then hit me.
Achievement Hunters "Fails of the Weak" on Halo Reach has to be the most hilarious compilation of deaths I've ever seen (most of them are accidental, and I don't think you have to be a halo player to appreciate it).
However for my own experience the first time I got handed a PS2 controller was when GTA Vice City happened to be in the console. First thing I did? Took a scooter, struggled to control it until I hit a fence and went flying off and into the water. There was no swimming mechanic in that game so I just ended up drowning. All happened in less than 10 seconds. My gaming skill has improved vastly since.
