I have no idea how many times i played the final boss in Tales of the Abyss. Because of the relationship he has with your character the whole having to fight him thing just seems so epic and the fact that he and your characters are actually talking to each other as you fight puts the finishing touchs on the whole thing. also hes quite challenging until your characters hit level 80+ (i actually lost to him quite a few times after the first time i beat him). But then again i could just be a unbiased opinion on the whole thing since i see Tales of the Abyss as the best RPG there ever was and more than likely ever will be

Kingdom Under Fire: The Crusaders and it's Sequel, Heroes, just have too many "intense" levels to choose from. On the harder campaigns (especially in Heroes) pretty much all of the levels are "intense".

One that sticks out to me is the level where you have to attack a human supply depot during Cirith's campaign, and your commander leaves near the start. Bloody hell, SO MANY HUMANS. In the end I just legged it to the escape point. Barely made it out alive.

Fun fun.

- Numerous occasions in Amnesia: The Dark Descent. In fact, pretty much any time a monster catches sight of you.
"Oh fuck oh fuck that noise! I can hear it - it's right behind me!"
"Run, damn it, run! Left! Dead end! Turn arou... oh god it's right there! Run!"
"Door! Open damnit... oh fuck I can hear it getting closer, why won't this door just bloody open?!"
"Oh right, I have to pull. Open. Run through. Turn. Close it, quickly!
"Oh god, it's beating down the door! The wood is splintering."
"Only seconds left. Hide. In there. Now!"
"I can hear growling. It's through the door."
"Please-don't-find-me-please-don't-find-me-please-don't..."

Yeah. You get the idea.

Also, Mirror's Edge had some heart-pounding moments. Tearing along at breakneck speed, leaping up walls and vaulting obstacles while bullets fly past your head and heavily armed men burst in from every direction. Good stuff.

Oh yeah, and the final mission in Mass Effect 2. Knowing that any screw-ups on my part could mean the permanent death of a beloved character made the whole thing ridiculously intense.

Plenty of others...

The last part of "The Parish" campaign in Left 4 Dead 2. Damn, that bridge action was intense. It makes a perfect climax for the whole game.
