World of Warcraft. Way back in The Burning Crusade, in Black Temple and at Mother Shahraz. We were partway through the fight, everything was going fine, and from my position I see two people with the Fatal Attraction debuff running the wrong way.

I can't remember exactly how it got to it, but from that moment all I can picture is massive shadow spell particles, ventrilo being spammed with "What the fuck?" and watching in a state of pure disbelief as 25 corpses fell from the sky and landed in a neat pile at the foot of the boss.

Then there was the time further back, when one of our healers was more than slightly inebriated.
To cut a long story short, a searing totem managed to spawn 7 sets of Hydross adds before everyone died.

Honourable mention goes to every single time that flame wreath was set off.

I cant think of many, but I shall tell you a tale of events that happened recently.

I was healing in 10 man firelands as a holy priest. We were fighting ragnaros in the first phase. There are a few abilities in p1 but none that are too challenging. Anyway I'm healing quite the thing, and ragnaros smashed his hammer into the ground (as he does periodically) sending a wall of fire to the left, right and infront of the hammer. The idea of course, is to dodge the fire. I was too engrossed in healing and didnt spot it in time to dodge it. Knowing I was about to be hit I had my mouse ready over healbot to cleanse the DoT it would apply to me.

BOOM it hits me, and I cleanse, ready to go on with the fight... or not...

It sends my character flying backwards(into the path of the flamewall again) and BOOM it hits me again. my health is dangerously low at this point (something like 1k)I use desperate prayer and I see the druid use nature's swiftness on me. Once again however,

I get sent flying backwards and hit for a 3rd time. This one kills me and blows my corpse off of the arena. Meaning my spirit of redemption was useless and I couldn't recieve a combat ress.

The raid wiped.

Basically every boss fight in Persona 4, simply because i refused to grind any levels whatsoever until the final boss and my final persona was some shitty non ultimate Priestess

But there was the one boss who had epic fire and ice attacks that i needed to fuse Legion just to get through that fight (it's strong against fire and ice you see)... That persona was never used ever again after that fight

I still owe my beating of the 5th boss to my final "you're playing on easy mode so be nice to the little retard" potion

Weirdly I steamrolled through Persona 3 on easy... And that's the one with the rage inducing friendly AI. Well they made up for that with The Answer i suppose (still need to finish that)

Not sure if there any catholics in the Forum if so i am sorry, but assassin creeds 2, fighting the pope, he falls on the floor and you stand there just kicking the crap out of him, i found it really funny. I am not a big fan or religion but even less of catholics. (again sorry for upsetting anyone if i did).

That made Fenris betray me and use the scythe ability and one-shot my entire party!
I had something like that happen. I was playing a rogue (I think) and romancing him, then that happened. My party was me him anders and varric. One second I was attacking the demon, the next I look down to see anders and varric DEAD. I managed to shred Fenris with rapid attacks and gulp a potion to take down the demon. Now, if I take him into that mission, I remove his weapon and acc's so he's not as brutally OP.

OT: Well, the only one that really sticks out is when I was a kid. My cousin let me borrow FF7, and I only vaguely remember the opening level. Fighting blue soldiers and blue...cat things. Then I remember fighting some giant red thing that completely owned me every time. 10 years later, I now know why that boss was able to obliterate me, and I wish to smack whoever programmed the warning in that fight upside the head.

Back when I played WoW, I remember getting a group for an outland dungeon. Botanica I think, the garden place in Kael'thas' dungeon. We didn't do too bad, I was an undead shadow priest dps, and we got caught up on that one druid boss. I forget his name, he's the one that summons adds while he heals. And for some stupid reason out group could not grasp the concept of Line of sight pulling the adds around the corner to kill them quickly, and thus we'd either die to the adds, or to him when he stopped healing. After about 4 wipes someone left and someone else came in to replace them, and we managed to get past him.

Also, NameIsRobertPaulson reminded me of one. I remember one of the first times I fought the final boss. She used that one move to turn me into a little girl, and while I'm slowly walking around she opens a hole in the barrier. Little kiddy form Bayonetta can't escape it's pull and bam, dead.

FF7 before i knew of final attack and knights of the round i was fighting emerald weapon that's the one under the water well it get to nearly killing him and boom he does a attack that does 9999 to each party member :( in case your wondering you need final attack with phoenix so you instant revive and knight of the round and mimic and w summon help also.

Oh and my first ever fight against a dark eaon in ffx I didn't know about them until Ifrit just appeared in front of me and wiped my team in one hit

The first time I fought Sephiroth in Kingdom Hearts with any idea of what I was up against. I got him down a bar or two and I'm like "Hah, I think I got this! Oh hey, this is new, why is he stopping? What's he saying? Why am I glow- OHSHIT WHAT THE FUCKING SHIT".

Fucking Sin Harvest.

The dragon from Demon's Souls at the first bridge. If you don't run inside fast enough you die, then you have to go all the way back and recover your souls.

Resident Evil 4, just about every boss gives you a memorable death but the chainsaw crazies were just full of awesome

Cloud was a pushover. Now, the "Mysterious Figure" and "Lingering Sentiment" from Birth by Sleep... Hoo boy. Where to even start with these two? Hardest bosses in the entire series.

In Runescape, for a certain very high level quest i had to fight a lvl 450ish opponent.

he 1 hit me through my armor and prayer and i just about put my hand through the screen

My one was I think our first time doing Blackwing Descent (in WoW) and it was on the very first boss, Magmaw. There were just the two adds in front of him left to pull before we went for him. Me being the tank it was my job to pull. So I cast Avenger's Shield... only to realise I'd miss-clicked. It turns out clicking anywhere on the floor in the room targets the boss. So yeah, no prizes for guessing how that one went. Fortunately my guild are a cool bunch and we ended up pissing ourselves laughing for 5 mins. I would get reminded of that at every boss encounter (sometimes when I wasn't even in the group, they'd just tell me) for a week or so.
