Real originality is pretty scarce these days, not because developers are losing their touch, games are just getting a little longer in the tooth and so most of the obvious routes have already been explored.
That's not to say it can't still be found though, Portal is based around a single original and pretty genius idea, and Demon's souls has some very cool orignal touches to it's tried and true fantasy action.

Hard to answer this really, as older games that came up with something original tend to be copied to death by newer games.

Story wise, if you know nothing about it ahead of time, Planescape Torment.

Biggest blindside from an old game in terms of plot would be Ecco The Dolphin.

Pure story I would suggest Longest Journey, some bits are obvious but mostly it is deeply involving.

Edit: Aquaria is really good too, maybe not plot wise but it is essentially Ecco The Dolphin mixed with Metroid style exploration.

Fallout 1. As far as I know, it was one of the first rpg's that wasn't class bound and had a turn base option in combat in a rpg (I could go on but just.. yeah.. fallout!!!)

I was about to post 'Okami', but that game reeks so much of Legend of Zelda gameplay that I almost can't do it with a straight face.

Otherwise, I'm not entirely sure if you can find a completely original game. My best bet would be Ace Combat IV. Flight sim, but with a story element. An actual good one, too!

Metal Gear Solid. I have never witnessed a game since that breaks the fourth wall in such a cool way.

OT: Easily Pathologic and The Void, both games made by the indie Russian developers Ice-Pick Lodge Studios. They are the most original games, and unfortunately one of the most underrated and overlooked.
No one apparently has played these two games over here *sigh*
Also Wikipedia classifies them as art games, and yes, they deserve that title.

Well when you play the game I often found the AI a bit silly and the lack of any interesting story annoying. In a game based on innovation I believe personally that you would need a good story to get people to buy into the new gameplay style. oh and the gunplay was like facerape from a head crab

Admittedly, it took me a second run-through of the game to know and understand the story (although this is because in my first run I just wanted to run the courses). To be honest, Mirror's Edge plot and setting could've been better presented and structured...

but personally, I found the idea of private law enforcement agencies countering the Runners with their very own Runners to be quite interesting. But hey, to each his/her own.

... you're not supposed to use guns, since they can be a disadvantage, as demonstrated by the disarming mechanic.

I always found Pikmin to be pretty unique and original. Maybe not as crazy different as some games but I really enjoyed it and haven't found anything like it game play or atmosphere wise.

The first 3-D zelda games, golden eye, & final fantasy.
These probably aren't the most original but they were set apart
for their originality and revolutionized the way gaming is today.
SO many games still use the principles set by those games.
