Concept: The teleport guns in WH40k that warp little gremlins into the targets body so they can bypass the armor and just kill them but gutting them up from the inside.

Execution: The gravity gun.

The hedgehog grenade on Resistance. It just sorta floats up to your eye-level, then blasts everything in the vicinity with sharp projectiles. Good way to pin a chimera to a wall in a funny way.

While the crowbar is awesome, the logo is in-fact the symbol for Lambda, ie wavelength, and the first game was set at the Lambda Complex/Black Mesa facility. :p

Cerebral Bore from Turok 2, I remember when that game first was released it was the goriest thing I'd ever seen.

It's not very inventive, but the idea of it really appealed to me. The papercutter blades of Condemned. Pulling a rusty dull blade off of the devices we've used in offices and classrooms all of our lives, and then defend yourself with it is a really cool concept to me.
