There are several actually...

Sly Cooper 2: Band of Thieves when your doing a tracking/pickpoketing mission where you know that the very moment your spotted that your happy arse is going to be plonked right back at the start of the mission.

My very first boss fight.

Having the tiniest sliver of health left during a boss battle and they have heaps left.

That part in Okami where your being chased by the Water Dragon and your swimming about an inch a minute.

On Far Cry 2, it sometimes would freeze during the load, and it'd get corrupted.

Anything where you are chased by something, like the guy with the axe in Cryotasis, the big monster (what ever it's called) in Gears 1 and plenty of other moments, sometimes I can't play the game because I'm being chased.

System shock 2. The level in the cargo bay when the pods start opening...
For the rest of that level I was so paranoid (especially since i'd run out of ap ammo)

In Portal when you go through the offices, Ravenholm in Half Life 2.
Beating the final boss of Unreal Tournament III on Insane.
The Great Gate and the events leading up to it in Oblivion.

Anyway, I'd have to say most 1v1s at the end of a CA on Quake Live can get very tense.

When the team says "Oh hey, letts split up, Me and the rest of us will go right wilst you go left alone", and since you can't say anyhting your forced to do what they say, then your team gets slaugtered and your alone forever.

Or in a horror game when the music stopps.

Or in Team Fortress 2 when your a sniper and you see the entir team commingat you (on the last cap point of dustbowl) and you have to take out the medics befor the go uber, then you die.

Playing FEAR 2 at 2 in the moring with a lightning storm out, then Alma gets all creepy and just as lightning strikes she jumps me, and also at that very moment my cat jumped on me. I pissed my pants and the cat got soaked, it was funny as hell though

Yes. A thousand times yes. Love it. Especially as it isn't pre-scripted, it's all AI and coincidence, which means ANYTHING can happen. That's what I call infinite replayability!
