I'll add a tip: Although the game was released after Doom, it didn't use the Doom engine but a modified Wolfenstein 3D engine, which somewhat diminished its success.

Those are my three guesses.
Heretic and Hexen used the Doom engine. The game we're looking was released in the same year as Heretic though. Spear of Destiny is a really good guess but it's released 2 years before this game.

Another tip: Just like with the Doom-based games you could select a difficulty setting. Here are those from this game: Corporal, Lieutenant, Captain, Major, President. The "President" difficulty was only available on the CD version of the game, not on the floppy disk version.

Is this shooter historically accurate, or does it have nothing to do with actual history?

I'm pretty sure that's NecroVisioN. And it's a "World War I alternate history survival horror first-person shooter". So not quite historically accurate :) I've already posted two screens recently so someone else can post the following.

And I promise not to go that deep into the 90's anymore for at least one or two screens :)
