I think No Hope and BSAA Helper for Mercenaries are just rumors that came from references that people found by poking around in the files. I've finished all stories on Professional and did everything in Mercenaries, but neither No Hope nor BSAA Helper have been unlocked.

Seriously, however: On pc Skyrim, put your level all the way up at the very beginning of the game, but not your skills. Master difficulty. Everything in the entire game can now kill you, basically. Or if that's too intense, put your level up to ~10 at the start.

Just finished the "EpicNameBro" challenge in Dark Souls last month (though I added a couple extra criteria of my own):

*You cannot level
*You can only use your starting equipment, rings are ok
*Can only upgrade through Andre (in other words, your starting weapon can only be normal, raw, divine, or occult)
*No healing, ever (the cleric's Heal miracle is the only exception)
*No covenants (cleric and knight just stay in Way of White)
*If starting as a pyromancer, you cannot upgrade your pyromancy flame
*No summons, ever
*No Head's-up Display

Did this with a wanderer. Getting used to not having a HUD was a chore.

Also, pick a Touhou game, any one will do. If that alone isn't hard enough for you, try to do a 1cc challenge on its hardest difficulty.
That jazz is insane, yo.

OT: Beat ALL the campaigns on Left 4 Dead on Expert using only pistols (or a melee weapon for LFD2)

Magikarp Power - beat an entire pokemon game with a Magikarp.

Food, glorious food - only ever restore HP with food items in Fallout 3.

Backpacks don't exist - Finish Mass Effect 1 with the starting equipment.

The logical choice - finish Dead Space/Dead Space 2 with nothing but melee attacks (boss fights excluded).

Humiliation - In the final fight for Halo: Reach (against the 8 or so Zealots), use nothing but melee attacks to kill them all on Legendary difficulty, solo.

And, as a rare example for a MMO impossible challenge -

Fuck Twinking, I've got real skill - Finish a Battleground in World of Warcraft at level 19 while earning at least 10 Killing Blows and capturing at least 1 flag. You cannot use speed boosts, get someone to heal you on purpose (a random healer in the BG can heal you, but you can't go in with a healer buddy) and can only use White-tier equipment purchased from vendors.

Considering how easy it is to get twink gear now and how many twinks exist, this might be the single most difficult thing in the entire universe.

Without the realism mod, beating fallout 3 with the 10mm pistol and some vault armor shouldn't be too hard unless you put it on the highest difficulty. Even then It'll probably still be doable.

I suspected as much, the Glock 19 is good in my realism mod, just it can't keep up with automatics and rifles.
