"I hate Nathan Fillion".

While I can't imagine anyone saying it, I would imagine it would piss everyone (including myself) off.
I hate Nathan Fillion [http://nextlol.com/images/2273-come-at-me-bro.jpg].

Naw, I'm just kidding. Hold your pitchforks.

OT: Basically what the OP said.

"Grow up" does really set me off, although I haven't heard it being said in a long time.

And telling me I wouldn't understand also has this effect, because usually I feel as if the person is trying to call me stupid.

I have too stick with "whatever" I've pissed off so many people with that simple word.

I can't really think of any phrase that annoys me at all.

"Whatever" is the most disrespectful thing anyone can ever say. When used in the context of an argument, it translates to "I have no legitimate rebuttal to what you have to say, but I am too egotistical and obsessed with myself to admit fault, so I'm going to turn around and walk away because nothing would make me angrier than to say 'you're right'."

"You're wrong" when something I say is an OPINION.
Also, when people repeat "No" over and over. "NO NO NO NO NO N-" *Punch*
Being an opinion and being incorrect aren't mutually exclusive.

For me it's probably `What are you going to do about it?`, when said in a snarky kind of way.
Someone's getting bitten, that's what I'm going to do about it.

Also, `I don't like it when girls x`. I don't give a fuck.
One dude was bitching at me once because I swore (at a general shit thing that everyone else was swearing and moaning at) and he simpered `Oh, women shouldn't swear. I don't like it when women swear.`
To which I replied `Fuck shit buggercunts kiss my arse`.

Captcha: I love you.
Steady on, mate.

Yeah I know that, but imagine this Scenario.

"I like Call of Duty 4."
"No, Modern Warfare 2 is the best."
"But I think the mechanics and maps are way better in that game and it has more balance"
"No way, Modern Warfare 2 is way more fun"
"I like COD4 better"
"Well you're wrong then, need to play MW2 more"

Something like that pisses me off.

"No offence, but..."

Is to a conversation what adopting a boxing stance is to a pub-confrontation.

The ultimate question: "Why?"

Because it can be repeated infinitely until you arrive at a point where circular logic is the only explanation.

It could drive a person mad.

"Who would win in a fight, X-character or Y-character."

This one statement is one of those statements that will almost always spark some pretty insane arguments. Bonus points if trolls come in and start messing with both sides.

When I was growing up it was my mum with:
"You've always got to have the last word, don't you?"

"Well, no, bacause you field that exact same line EVERY SINGLE FUCKING TIME!!!!"

But I suppose religious people who blindly deny science, people who are dead set against gay marriage despite every argument against it being WRONG.

And from work, "Last of the big spenders" (Which I think I hear on average about once every hour or so. After the first few thousand it's impossible to even fake a laugh, I just look at them like I don't get it), "How else am I going to get it home, magic?" (In response to me asking if they want a carrier bag, like I've just kicked them in the shins and told them they can't prove I did anything, so they'll have to be immediately hostile to me for doing my job.)

EDIT:

Not a bad display of impromptu swearing. Incidentally, do you have any kind of accent? It would make it considerably more awesome to imaging that being said in, well, any accent, really.

For me, it would probably be when someone says, "you like anime? what are you a narutard?" or when someone says "arrow to the knee", my reaction woud be this:-

Then when I regain coniscious, all of a sudden, I wake up to this:-

"first world problems"
That phrase is just so fucking stupid. It's such a blatant attempt to guilt-trip me, and I HATE being guilt-tripped. Not to mention, "despair" is completely subjective. If my cell-phone breaks, that's shitty, and it's not less shitty just because someone might have had a supposedly worse day. It's dumb, and the people that use it to bolster their arguments are twats.

Oh. God. This. Thank you. Read the comic. Summarizes why this phrase is so stupid. When you do it back to them you can literally apply it to ANYTHING no matter how brutal.

"Oh your dog died? Someone in somalia lost their parents idiot. First world problems."

People who try and downplay your suffering by comparing it to someone elses are morons. It doesnt help. At all.

"Happy birthday"

... Yes, seriously.

...

I have issues.

Oh, and "Calm down.", "Stop being so ANGRY!", and other variations of those phrases. Especially the latter. It's like pissing in someone's ear, then waving your still-pissing penis at them and saying it's raining. While take a dump in their pool.

EDIT: And 'It's worse in Africa.', 'Somebody else has it worse than you,' etc etc. Sure but that doesn't whatever I was complaining about doesn't SUCK

When you are obviously injured/in pain, and someone asks "Are you okay?" I understand that the person is worried, but I hate hearing stupid questions like that, especially when I'm in pain.

"It's not such a big deal, stop causing drama."

That phrase is all sorts of horrible. It means that they don't care about your concerns, they don't think your feelings matter, they force you to keep your feelings and legitimate needs to yourself or risk becoming the "drama queen". It pushes you down and it silences you. "It's not a big deal" is a phrase that can ruin your life. It is a big deal. If I say something is bothering me I mean it, and it's not ok for you to dismiss it.

This article explains the potential damage of this phrase and this sentiment incredibly well with a particularly alarming example:
