Age of Conan. In that game, if you die you respawn at the "nearest" spawning point that you've visited. Except that point was one I visited with a party of high level characters.

So the next time I died, I respawned all by myself in an area designed for a team of high level characters. I never made it out.

New Vegas. Quicksaved just before a family of deathclaws attack me. I survived, but at the cost of over 40 stimpacks, broken unique armor, and almost an hour. After the end of that encounter, I gave a loud "UGHH" as my battle cry. A few minutes later, I realized it was all for naught, for there was no valuable loot in that area.
Wait, the only three places shit-full of deathclaws are Sloan, the Deathclaw promontory (the lil'island) and Dead Wind Cavern. Dead Wind and the pormontory have awnsome loot, so...

Wait, it took you 40 stimpacks and a Unique armor to get through Sloan? How?
It wasn't Sloan. It was the Trainyard northeast of NV with the building that has 2-3 ants inside. I was at a very low level then.

I quicksaved in the kitchen area of Max Payne.
About a quarter of a second before a scripted fire extinguisher flies out of a corner. I ran into that and died so many times.
It was HORRIBLE. And I was about ten minutes into the level, so I didn't want to restart, so I kept trying.
Over.
And over.
And over.

Another one was waaaay back in Call of Duty. I quicksaved in a building that was being fired at by tanks. I had a sliver of health left, and as I saved, I was shot by a tank.
This happened a good thirty times before I managed to avoid being killed.

I was playing Dark Souls when I attacked a really high level friendly NPC. The game Auto-saved as soon as he started attacking me and the guy killed me in two hits. Game immediately reloads and I die again. I try to kill him 10 times before I decide to give up, but you can't pause in Dark Souls. It took me three tries to open the menu and hit Quit as the guy killed me from behind the menu. I finally had to turn off the console, delete the save and start over. Miserable.
I still think that, practically, leaving out a pause function in a game is just an absolute troll move. Especially since the ability to pause has been a staple of even the most difficult games since the NES era. I dunno. Just my opinion.
OT: Oblivion. I saved in an Imperial City guardhouse, where I was sneaking around (whilst having an enormous bounty because, well, that's how I roll), only to have a guard spawn in (as they do) about a tenth of a second afterward.
STOP RIGHT THERE, CRIMINAL SCUM!
And then I died.

In Skyrim, I was walking up on a mountain to where a dragon was, along with one of those stones which you get a word from.
I saved and right after I die.
There was some kind of magical artifact there that casts spells that kill me instantly...

I had one in Max Payne 2, I was replaying the game whilst listening to music. I didn't get far as I ran to a door in the first level (about 3/4 of the way through) and saved next to the door before I breached it. If I wasn't listening to music I would have heard the conversation the bad guys were having about blowing the door off :( Didn't restart the mission just gave in.

OP, you sound like an Elder Scrolls novice. No offence intended!

I learned the hard way, back in the Morrowind days, to create a new save every time.....and save every ten minutes!

When I was playing Black Ops on veteran for the achievements (I'm an achievement whore, I know)I had an auto save mid fire fight. I was in the middle of a large square room, my primary weapon had 2 bullets in the clip, my secondary weapon was a crossbow. There were 3 enemies directly in front of me, and one ahead and to the right. Behind me, there was 3 more - but not directly behind me. They were about twenty feet behind me, and I had a pillar between me and them.

What I had to do everytime it loaded was spin around (thank god for 10 sensitivity!) sprint over to the guys behind me, knife one, swap weapons, kill the guy standing next to my knife victim, spin around, kill the third guy (who was in a hallway heading into the room. Not sure how he got behind me that way :S)

Then it was a simple matter of using my crossbow to peg one of the guys in the group that I loaded facing. Took me forever to beat that level on veteran *just* because of that one terrible autosave. I was about 2/3s of the way through the level, and I really hate backtracking :p

I accidentally double-saved (writing over my back up save game) on the first X-box Ninja Gaiden with a tiny little bit of health left, no potions or talisman of rebirth and no currency to buy healing items, right before a mini-boss battle followed up by a trap and platforming heavy area. I ended up wasting hours of work (this was on level twelve or thirteen if I remember correctly).

So, I was just playing Skyrim (AAAAAAAAAAHH ANOTHER SKYRIM THRE- NO! SHUT UP!)
and I know the value of quicksaving.... every 30 seconds.

Anyway, I was fighting this dude, and another dude. So I smash them into the groud, low on health, and so I quick save.

A dragon lands in front of me and eats me. In one of the pretty death cams. Yup, I get picked up, thrown side to side then flung into the air.

When I reload, I pop back with a slither of health, Mr Dragon-Nom-Face lands, and eats me. Nom nom nom.

Shearpoint. I see the dragon just chillin' on the Word Wall. I zap his sorry ass back to wherever Aedra come from. Go to the word wall, get THREE WORDS OF POWER,HOLY CRAP! SAVED! Turn around...And a lich. With a fireball mid-flight. Well, fuck.

I tried so many times to get away. It just wasn't happening. So,sadly,I had to turn on god mode, and run away. When I got back to Whiterun,I turned off god mode.
But why don't you have any health potions?

Seriously guys.
Always, always, always have enough health potions!

I usually carry 30-40 of them myself, though of various strength.
One can never have enough health potions.
Never!
Sorta hard to heal when you're one shot murdered by a fireball.

But does not Skyrim pause when you enter the menu?

Unless you get one shotted with full health of course.

I was playing Star Wars: The Phantom Menace when I was about 5 or 6. I got to the Gardens of Theed (if I remember the name correctly) and was in a fight with some droids. I had the minimum amount of health that you can have without being dead, and a laser shot was literally in mid-air flying towards me.

For some reason, saving over my only file at this point seemed like a good idea. I never got any further.

Phoenix Wright: Justice for All. Last case, very near the end of the trial. Pumped up, very excited, gonna get that case done.

Then I noticed the battery was getting low and my last save was about 45 mins prior, so just to be safe, so I popped up the save menu, click save... then the battery died, as it was saving. Whole save file was gone. Had to do the case from the beginning.

That is my worst save.

OP: Don't know if you're on PC, but if you are you can reduce the time between auto saves from 15 mins to whatever you want, even every 30 seconds if you'd like.

As for the discussion, I know I've done this type of thing but I can't recall when.

You'd think it was safe to save after the "course clear" message wouldn't you? Think again. Skip to 7:20 if you don't want to watch this the whole way through.

This is an evil romhack of Super Mario World called Super Kaizo World. The headblock just before the exit contains a P-Switch which turns coins into blocks. If you don't use it, Mario will fall through the coins in his victory walk and die. Now you know what a Kaizo trap is.

I remember one in Thief 2. The level was called "casing the joint" and I had to infiltrate a mansion without being seen or knocking out any guards. There was one point where I quicksaved about 1.5 seconds before a door behind me burst open with a patrolling guard. There was nowhere to run or hide and the mission would fail instantly when I got seen.

Luckily, I could still use my flashbombs, and after trying 20+ times to blind him and get away, I succeeded and completed the mission. I had spent probably 2 hours in this house trying to not get caught. I didn't want to start it over again.

Playing "Legend of Dragoon" on the old PS and got the save prompt at the end of disc 2 (4 disc game). So I saved. Loaded into the town on the new disc. Bought some stuff and saved. Move to the edge of town and felt a boss battle coming on and saved again (3 different saves).

Boss battle started and it turned out this think took 80% reduced damage and did 200% damage to all characters in Dragoon mode, this wouldnt have been a problem except like in FF8 GF's Dragoon mode levels independently to your character and not at the same time but up till this point, like FF8, there had been no incentive not to use the retardedly OP dragoon mode so my normal modes were crap. Tried to leave town the other way, cant, not allowed to move backward through discs. All 3 of my save filed were now in the new town.

Never did kill that boss. Shame, was a fun game till that point. Its a testament to how much Id used and how OP dragoon mode was that despite these weaknesses I managed to get the boss to sub 10%.
