Spiders.

The reason i'm not scared of Spiders IRL is because they don't attack me (unprovoked....till now). In Games, they usually do, and are usually pretty big, and are often still small and agile enough to be hard to hit, so...

Pretty much the whole creature making a sound that doesn't sound human but you cannot see them nor knowing where they are.

I'm with you there. As someone with a lifelong fear of underwater creatures, nothing freaks me out more than being attacked by one in a game. Underwater sections are tense enough anyway, trying not to run out of oxygen, the last thing I need is some scaly dagger-mouthed thing coming out of the murk straight at my face.
Slaughterfish in Elder Scrolls especially make me want to spend as little time in the water as possible. I remember a quest in Oblivion where some old man wanted me to go into the lake and collect twelve slaughterfish scales for him. My thoughts were something like "Go fight an evil fish, on their home turf? Twelve times? Fuck that, get your own damn scales." And then I ran into the giant slaughterfish on that thieves guild quest... just thinking about it gives me the shivers.

I think the only thing that has really scared me in a game is Pyramid Head. It was the scraping noise he made when he dragged the great knife on the floor that really freaked me out. And if I hear anything that sounds even remotely similar to that effing alarm, my heart starts beating like crazy...

A lot of things in Silent Hill really =S Pyramid Head from SH2 most of all.

There is a section over half-way through where you're in some kind of sewer/flooded area (I remember ankle-shin deep water), and to this point have been given absolutely no indication or clue that he is close by. I could hear splashing but there were other normal enemies around so I didn't consider it too unusual. Bear in mind it was also very dark.

I was running up to a two-way split in the path (I could go straight on or turn right) and saw his silhouette in the distance. I walked closer very slowly and jumped out of my skin as soon as I realised it was him! I backed away slowly at first praying he hadn't seen me and took the other path which led to a small room with a ladder and another path that loops around to where I was (i.e. he is down that path). Up the ladder was a dead end which meant to I had to go back again. The problem was though that I didn't know which path he was down (if we saw me earlier then he would have followed me, if not then he was where I found him before).

So my best strategy was to WAIT (in the friggin dark since torchlight would immediately give away my position to him) at the foot of the ladder, listening to his footsteps and watching both paths at once until I saw him. I was absolutely f*cking petrified!

this might sound silly, but what really terrifies me are sharks. For example, in the original crysis I only tried to swim far from the Island once...never again. I don't really know why I am terrified of them, not even how considering I have never seen a real life shark up-close, but I can't really handle that.

Headcrabs always get me, I mean they are not even that much of a threat but the way they move and leap right at the screen *shudder*

Then in half life 2 they just had to make the poison ones who just hearing the sounds they make idly now makes my skin crawl

In a game so full of symbolic messages and tragic stories, the scariest thing is the atmosphere in general. It's the constant "Oh hey listen to the pretty music and look at beautiful Clock Town! But hey, you better hurry up, CUZ YOU'LL FUCKING DIE!!!" feeling that makes the game so impressive.

I also cringe every time I see the Butler's son...

I am lucky to have never come across a game featuring woodlice. Luckily they all feature spiders, which I quite like. Especially this guy.


The dark ones from Metro 2033 creep me right out, but only after I started reading the book, but had to stop because they gave me nightmares about dark ones.

Apart from that, some of the random tentacle sprouting that the flood did in Halo was a little disconcerting, but overall I found them more irritating than creepy.

spiders and water creatures like almost everyone else has said. I don't mind the slaughterfish in the elder scrolls games oddly enough but things like the great white in Scarface for the xbox. Also those frostbite spiders in Skyrim, I normally play the game first person but I see those and zoom out as far as humanly possible. I still remember when during a quest for the companions this guy you're with says he can't go on because he's afraid of spiders. It pissed me off because I was probably even more scared than he was but I had to complete the quest.

Emptiness and quiet, especially when there should be enemies around. Moving slowly, trying to watch everywhere at once. It doesn't even matter if it's in a game where I can curb-stomp the enemies. Put me in a space and don't throw anything at me and my heart will be in my throat every time.

The Boy of Silence that everyone is talking about in Bioshock not only made me jump on my first playthrough, it also made me jump on my second playthrough, and that was actually even after watching the gameplay video that someone linked in this thread. *headdesk*
