Try the shark in GTAIII. That will scare you, even though it's really just there for kicks.

I remember once it got me even when I was on top of a boat. After that, I stayed the Hell away from the ocean in that game.

Sometimes I get so antsy that even Portal is to terrifying to be enjoyable. It's the creepy silent protagonist thing. :(

Fallout 3 is also sometimes terrifying where it should not be. I don't think the feral ghouls are supposed to scare me as much as they do.
If feral ghouls creep you out stay the hell out of the dunwich building.
OH MAN. OHHHHHHHHHHH MAN. I wandered into that, and couldn't get out fast enough. Goddamn creepy flash backs. I thought this was supposed to be a happy game. :(
If Feral Ghouls scare you, I suggest you make a melee-based character and enjoy smashing them up with a sledgehammer. That generally removes any feelings of fear. Haha.

no one mentioned zelda?

OOT was nightmarish...
Yes, the part right after you change to adult link and the city is full of those zombies. When I was little the sight of one of those things terrified me.
Spirit Temple.

*shudder*
I am STILL scared of the re-dead in Zelda, I remember the first time I ever saw one was in some tomb in the graveyard where you get the sun song, I didn't know what they were and so I hit it with my slingshot from afar and it ran up and started choking me!! I turned the game off!!! I used to get my dad to run past them on the way out of the temple of time for ages before I picked up the nerve to do it myself. Those things are FREAKY!

(theres also a hidden grotto in Kakariko village near the entrance I discovered by accident bombing cuckos - totally random, no ring of stones or boulder- I never discovered what was actually in that chest because there was a redead down there. again- I was so scared I paused and saved and then reloaded)

Metroid Prime. I had already played through the game a couple of times, and this time, I really wanted to get into Samus' perspective, and while her suit is pretty cool, it probably doesn't have an I-pod in it. So I turned all the music off, and only had the sound effects on. Holy crap, that game is freaky. Without the background music, you hear these noises as you're walking through corridors and you're all like "What was that?" Seriously, try it some time. Silent Hill didn't freak me out. Metroid Prime freaked me out.

It's not their appearance, it's how gorramn fast they are! I like to pick things off at a distance, and they like to close space. :(

In some games, Fallout 3 is a good example, random junk is strewn about the hallways.

Sometimes when I'm home alone with my sound system up, drunk at 4 am, and I my character inadvertently kicks a tin can or skeleton, that sudden scraping sound is the fucking scariest thing imaginable.

The first time I played Bioshock, it scared the crap out of me. Probably had something to do with me making the darkness far too low. Also the fact that the dead bodies kept twitching after they died... (shudder)
Strangely enough I usually get scared or creeped out at games that are scary, but Bioshock never actually scared me... And frankly, I don't know why. Maybe I'm weird or something.

Metriod Prime. Prime 2 also did this. There's just this sense of... Creepiness in the games. You're the only one on this hostile planet going from location to location finding stuff... It just had a very suspenseful and creepy vibe to it... I don't know why this game scared me and Bioshock didn't... Like I said, I'm weird.

The Re-Dead were terrifying, but you know what scared me more? That fight you had to do with this monster that had no hands but the hands come up from the ground and then he would slither over to you and his mouth would open really wide and he'd bite the shit out of you. I have no idea where it was, but man that thing gave me the creeps.

Also, I recently played through Dead Space and spent more time unloading clips into my shadow than anything else.

I had a creepy moment today in Oblivion. I felt like murdering some poor sop in his bed in Skingrad, so I began my rounds searching for a good home to break into. Thing is, when I come across one, and successfully infiltrate into the home, the music goes quiet as if you're in a cave.

And then I heard the plip plop sound of rotting flesh hitting the floor, and I became aware of my surroundings. I had stumbled across a haunted home.

Definitely one of the best moments in the game so far.

T-Rex. The T-Rex in any game will scare the crap out of me. Doesn't matter if the same game has an even larger mecha-lizard that shoots laser beams and will feed on your soul. The T-Rex will scare the ever living crap out of me regardless.

Parasite Eve. This was done back when survival horror was a new genre, with not a lot of proven strategies. In addition it was done in PS1 graphics. Oh, and the enemies were mostly just mutated animals, sort of like what RE series keeps forgetting isn't that scary. You also play as a cop who on the very second level gets access to both a rifle and a grenade launcher. Ammo's also in good supply. How bad could it be?
Long story short, I don't think I'll be trying to cure my insomnia now. The result would just be nightmares for a long time.

(Yes I am somewhat cheating because it was indeed a survival horror game, but all things seemed to point to it not being that scary. I was dead wrong on this one)

Babies.

Well, children in general, but babies totally freak me out. Especially mutated or deformed babies.

I am genuinely acrophobic, and even when I play games heights can sometimes terrify me. I remember in Half Life 2 there was one part of the game where you were on this big bridge or something, with girders and whatnot that you had to walk across, and it was legitimately hard for me to cross.

But the Spirit Temple? Really? What about the Shadow Temple or under the well in Kakariko Village?

when I was a child the redeads in "Ocarina of Time" freaked me out so bad I went through most of the game without ever getting the lens of truth/song of storms/ or setting foot in the shadow temple (strangely now when I replay the game I always go to the spirit temple before the shadow temple.)

and upon digging up sonic adventure 2 I find myself fairly creeped out by Gerald Robotnick's rant about how the human race betrayed him and how it echos when as a kid it never really bothered me "all-all-all of you ungrateful humans--who took everything away from me! will feeel my loss.. and despair-spair" (read that out loud in a staticy voice and that's what the rant sounds like... plus during the scene it shows a video of him sitting face-down in a chair that reminded me of "The Ring" which was one of the first horror movies I ever saw that scared me)
