When They Have More Plot Holes Than [Insert something with tons of plot holes, ex: " a Micheal Bay movie"].
I can think of a couple "Hugely Popular Realistic" shooters that fit that bill.

Things that piss me off in games?
hmmmmmm.....Yearly releases, Overpriced DLC, Good games getting thrown under a bus and bad games getting god like status. There is one more but really important one, but it unfortunately escapes me for the moment.

Unskippable cutscenes
Forced stealth sections where you're not allowed to be seen at all
Games with save points instead of a "save whenever you want" type of save system
Enemies with regenerating health
Enemies that keep spawning until you leave the area
Players that take online gaming too seriously and insults newbies
Copy-pasted enviorments/buildings/NPCs in large game worlds

Long unskippable cutscenes or training. Especially at the start. I will randomly make new games before even completing a game so I don't want to have to go through the same damn cutscene again and again, same with the training.

Any enemy that heals or regenerates. You guys gave us a health bar, now abide by it. Enemies healing takes away all strategy, because you have no idea when it might heal again, and how long you'll have to fight it, and what do you do, do you conserve MP, what...?

Characters that are badass until you get them on your party. No, I want to use Magus' Dark Matter NOW. I don't want to wait for him to level up. He used it on me in the boss fight; what's your problem?

People dying in cutscenes when regeneration is possible in the ordinary game. You mean I've been getting burned, crushed, slashed, and otherwise beaten to a bloody death over and over, and one sword to the face is going to kill me in a cutscene? Excuse me?

I have a special place in my heart for Persona 3 and 4's tendency to do crap like not let you control your party members (P3), and killing your entire party when your main character dies. I HAVE REVIVES WHY CAN'T YOU USE THEM ON ME.

Baldur's Gate and Fallout 2 earned my hatred for having me miss all the time in combat. I'd rather do 1 damage every time, than miss half the time and then do 2 damage. It feels less like I was screwed over.

Unskippable cutscenes. Not being able to save whenever I want. Being able to take both sides in a morality meter (Mass Effect, I'm looking at you.) Always having to be either AMAZINGLY GOOD or AMAZINGLY EVIL to get interesting stuff. (Every other game with moral system, I'm now looking at you.)

Uninteresting story in a game were the story plays a huge part.
A fighting game were the fighting is boring.
Something like this.

People spamming a glitch in a MP game.

Lost forevers

People who think its acceptable to throw around death threats because someones new at a game.

Being kicked from a server because you actually manage to be good in a game.

Oh i found something else that piss me off to no end: WHY THE FUCK, DOES CAPCOM CHANGED THE FUCKING THEME AND MENU MUSIC FROM Street Fighter 3rd Strike ? It was good the way it was before, now it sounds like something out of an MMA show. It doesn't fit at all. Not only that, but it's the only song playing in the online lobby, and it never fuckin stops (i know there's an option to put the old one but i don't know if you can do that on XBOX).

losing sight of the overall goal. Most role playing games have this problem, I get interested at the beginning, then faff about for so long that I can't remember the overall goal, then I get bored and leave.
Final Fantasy A2 is probably one of the biggest culprits of this problem. I've spent well over 100 hours on the game and at some point, I was unsure of what the plot was anymore. It's one of the few FF titles that is not know for its storytelling, which is one of Square Enix's strongest selling points for a game.

Action buttons. They aren't all bad, it's the first thing that comes to mind. I just hate having to hit a button everytime I wan to go up a later or pick up ammo.

Kingdom Hearts Secret Bosses

Or God-like secret bosses in general. These bosses are meant to be challenging and all they turn out to be is frustrating. One-hit kills, ridiculous ranged attacks, large amounts of health or armor, and no definite strategies that can defeat them. You'll spend more time dodging than you will attacking, you constantly have to watch out for your health than the boss, and the chances of winning are completely random. These bosses do not stick to a routine throughout the fight, and instead will either be incredibly distant with attacks or mercilessly killing you on the word go.

Give me bosses like Kurt Zisa, that relies on learning and avoiding his attack patterns, Phantom, that introduces a unique and interesting fight, or the Ice Titan, who provides a completely different experience to standard fights (being frozen, and reflecting all of its projectiles a the precise moment). Fighting a couple of bosses that can kill you with one hit is not challenging at all; especially when you continue to do it over and over again.

Timed sections.
I can't remember ever failed during such an event in games, but I still hate them...
Major offender; Dead Rising series.

Oh man do I hate hate hate having to do so BY THE PIXEL!
Teammates running into heavy gunfire = BOOM! DEAD!
Obscure objectives that you need to figure out to continue in an action-based game.

"Competitive" online gaming communities, seriously try playing LoL HoN or Dota as a newbie for a few hours (be prepared to be abused for those few hours).
To be fair it doesn't matter too much if you're new or not, I've played something like 100 games of LoL and I still get abused, the majority of the community is just shit. Though I think new players are copping a flogging so much these days because of so many people having smurf accounts and flipping their shit at new players for not doing something a player with experience will do.

I recently bought Burnout Paradise again and planned to plough through it. As I reached C class, I wondered why any person would ever not play this fantastic game.
Well, two massive, MASSIVE dick moves came back into note after this. My car suddenly became hugely flimsy during races; one tap of traffic and I'd catapult across the map.
Second, WHY THE FUCK WOULD YOU SPAWN TRAFFIC THAT SWERVES INTO YOU ON A TIGHT DRIFT ROUND A MOUNTAIN HOLY DICKING GOD

Other than the occasional spastic Burnout moment, doors that only open one way are another. I was disappointed when Skyrim, the ultimate go-anywhere do-whatever adventure, didn't let me just kick doors and chests in.
