The problem with most of these ideas is that they, as TV Tropes calls it, "cross the line twice." They cross it the first time by being utterly tasteless, then they cross it again by being so tasteless that they're no longer offensive but, rather, hilarious, even for those people who should be offended by them. This is how they get away with anything that they show on Adult Swim. Toning those shows down would only make them more offensive than they already are, so they just keep going further over-the-top.
So how's this for my idea: a World War II shooter where you play on the German side. In the campaign mode, not just in team deathmatch. And they're portrayed as the good guys. Nothing simple. No baby-raping, or necrophilia, or bestiality. At least, not from the "good" guys. Instead, those traits are given to the enemies -- the Soviets, the Americans, the Jews, the Poles. A sizable chunk of your character's witty banter with his war buddies involves talking about "the enemy" in these terms. To have your character committing these actions would not only subconsciously tell players that the Nazis are evil, it would expose the fact that the game's sole purpose is to shock the player, both of which would lessen the impact of the game's subject matter. Instead, by telling the player that the "undesirables" engage in these acts, the game is telling the player unironically that the Nazis were the good guys during World War II. And that, my friends, is far more offensive than throwing dead babies and gore and nudity and little boy booty at the screen.
