When I was playing the Fallout 3 add-on 'Point Lookout'. I was tasking with retrieving a book from the Swampfolks place of worship. I kill three swampfolk outside, and went inside the basement. Basically, the next two minutes was a stream of 'This is gonna suck' moments as I was expecting those creepy-as-hell Swampfolk to appear around every corner. Turns out, the place was empty. With a sight of relief, I pick up the book off of a pedestal. I walk forward three steps and a small army of swampfolk walk down the steps into the area containing the book. 10+ enemies, close quarters, severely underlevelled, no ammo. OH SHI-

Okami when you finally enter the Ark of Yamato at the end of the game and realize you have to defeat all the bosses....again.

Way too many. Off the top of my head:

Resistance 2 on the very first level where you shoot the goliath and then it starts to turn towards you. Slowly.

FFXII when I first entered the colosseum to fight Yiazmat. "Oh look, it's a ginormous, pissed off glowing holy dragon with 50 million hp that shoots tornadoes and lightning, scorches the ground with pentagrams and 1 shots everything. And the arena is full of traps. Greeeeat." Took me 11 hours to beat him, one sitting. Then I slept for 12 hours.

Thief Deadly Shadows, when I realised that the 5 huge golems that are chasing me are immune to everything.

Silent Hill 2, when you see Pyramid Head behind an iron gate, just standing there completely silent and motionless, watching you. And you're headed that way.

Pretty much any moment from L4D. My two friends and I fought a horde of zombies to get on an elevator. All of our ammo was used on the horde and we quickly entered the elevator. A bot, a friend, and myself were on board as a witch just walked up to the elevator's entrance. My other friend was downstairs.

In Warcraft III, I was recently having a match with some other Night Elf forces (controlled by the AI), with my level 8 Demon Slayer hero. Having lost any access to gold, I had utterly finite resources, except for my Moon Wells. I only had my hero, that's all. I had him fight their forces as they tore apart my base, taking him back to the Moon Wells to recharge his health constantly.

I actually managed to kill them all, and he'd reached level 10 by the end of it. But, after that, I realised my opponents had no access to gold, and didn't seem to be using their taken gold mine for whatever reason (the AI sucks).

So, I'd actually won with my one-man defence - they had no units to defend with, but I only had one unit with which to demolish both of the empty enemy bases. I was determined to do it, since they'd killed my ally. It would go on to take me something like 45 minutes, slowly chopping down each building. Truly a sucky situation, but you must earn your victory after pulling something like that off.

Probably in Dead Space, when you're walking down a big, wide corridor, with lots of places where the zombie aliens usually come through, and the suddenly the alarm sounds and the door gets locked.... There's only one thought in your head at that moment- "I'm SOOO fucked..."

Every time you have to reload. The AI have this special power of not appearing, you begin to reload, and they run out shooting.

Or running into an area, only to fire two shots and have to reload.

For me, COD 4 (yes, some people still play it): Hardcore search and destroy. My entire team is down (except me) with the enemy not losing a man. I'm on defense and the enemies plant the bomb. I head to the bomb site without encountering resistance. Thinking my pro sneaking skills had payed off I cockily started to defuse without looking around me properly. Time seemed to freeze as I saw the ENTIRE enemy team pop out from behind a dumpster in front of me. "OH SHI-" *5 straight seconds of gunfire*

In RE5, the cutscene in the butchers where Uruboros is first mentioned. It was then that I realised the whole game was going to suck, and it did.

EVE Online, on an epic arc, a mission where I'd already lost a cruiser before. Came back later with another cruiser, determined not to make the same mistakes I had the first time..... and I find out one of the NPCs is warp scrambling me around the point where my armour was at 40%. Pop goes another cruiser. Oh yay.

I was playing L4D with some "friends" on Death Toll, and we were prepping for the finale so I and a friend went over to that isolated shed to get some gas cans to form a perimeter. So while we are there the two people at the house decide to trigger the Horde and right as we get to the shed we see the Zombies approaching. I have a molotov with me and figuring out what my friends did I run up to the house, throw the molotov, light them on fire, and then get pounced on.

In any game: walking down a long hallway full of health kits and ammo. You ain't gonna like what's behind the next door.

In L4D, being the last survivor, and hearing the sound of a smoker, hunter and jockey being spawned all at the same time.

Dragon age, Arch demon fight.

I moved my party in, and instantly, the Dragon flew into the air, and landed on my healer, killing her instantly.

I had to fight the entire end boss fight using only health potions.
