I'm thinking of one in Dance Dance Revolution, of all places. Specifically, the extra stages in DDRMAX and MAX2. For the uninitiated, whenever you unlock an extra stage, your only choice of song is one insanely fast track and insanely hard chart. I can pass just about any level-9 (out of 10, at least on the old pre-X scale) chart without needing to "jump" on a DualShock controller and give my feet a rest, but the level-10 charts which are reserved for these boss songs are just beyond my physical ability. At least the later games let you choose a song for their extra stages.

The Demi-Fiend in Avatar Tuner.

Although the game is difficult, it's not unfair. If you're about to go into a room with a boss, it'll ask you if you're sure. There are plenty of health points and save points arounds dungeons, and most of the time, you won't need to grind too extensively.

By the time you fight this boss, you'll have maxed out all of your characters. In other words, you can't improve.
You'll have their best ammunition.
You'll have all of their skills.
And a solid party formation.
The game asks you twice if you're ok with it.

*Entire party completely dead with 0% chance of dodging or defending from attack*

My first time through Earthbound really changed my thoughts on it on my current run through. When I was younger, I had issues with:

Titanic Ant
Police Station
Trillionage Sprout
The Dept. Store
Kraken
Magicant
Giygas

The first four are a LOT easier than I remember, the dept. store wasn't as bad as I remembered. Kraken died on the first turn (Flash ftw), Magicant and Giygas were nearly impossible because I lacked one of three(?) choices for my defense items.

Basically Ness' Nightmare, and Giygas could randomly use an ability that is effectively Flash Omega. Which can make you cry (annoying), Confuse you (annoying, but usually over in one turn), numb you (not too bad if you have the magic to cure it), or about a 50% chance to instantly kill you. Having a Night, Sea, or Star pendant guarantee these attacks do nothing. When I beat the game the first time, I had no such protection.

Most recently? Hmm, I'd say in EU4 right when my truce runs out with the Ottomans, and along comes the 2x20 stacks of DOOM to crush my newly re-fledgling Byzantine Empire. And despite me having better military tech than them, a general that's almost as good and twice the amount of troops (from alliance with Poland/Hungary/Lithuania, and Bosnia/Serbia/Wallachia as vassals), the Ottomans crush everyone.

Your question doesn't specify video games, so allow me a tale of Friday Night Magic from this last week.

I'm running with a freshly made incredibly powerful White Aggro deck. Match 2 I go against one of the newer guys who has no chance of winning or any real clue as to what he's doing. So knowing he's pretty much already lost, we've agreed to just have fun with the game. I smoked first game. Game 2 he got an impressive field out, and swung them all at me. Being my cocky self, I let them through, knowing that I would survive it and destroy him next turn. Then he drops 2 Titan's Strengths. He wins.

Game three I had to mulligan down to 5 cards, and even then it wasn't that great of an opening hand. I pretty much said the title of this as I started my first turn. (I did end up winning, but it was a rough as hell game).

Remember Diablo's lair in Diablo 2? You have to active five switches, with tough fights all the way through, to activate the giant pentagram in the middle. Then there's an earthquake, and you hear "NOT EVEN DEATH CAN SAVE YOU FROM ME". And I go, "eep!".

Whenever I run into the Bloat or the Mask of Infamy in the Binding of Isaac. Seriously hate those two. Also maybe having to fight the Fallen on the first couple of floors.

Starting the side mission in ME2 where you have to kill FOUR MECHS before they destroy a couple of fragile crates and/or kill you. On Insanity.

Also, a certain part in the first episode of The Walking Dead Game Season 2. Those of you who've played it know exactly what I'm talking about.

When I picked up Enchanted Arms. And I was RIGHT. Also when I tried to beat Bioshock on Hard without vita chambers. I knew I could do it, but it would have taken so long I just didn't care past Steinman.

1. The Dread Lords showing up in a game of GalCiv II.

2. Most of the later missions in Homeworld 2.

3. Defending Carentan in Company of Heroes - that is a rare example of combined: this is gonna suck/be awesome.

4. Fighting Heihachi in pretty much any Tekken game.

5. Nemesis showing up in RE3.

6. A tank showing up in L4D/L4D2. (ditto someone alerting a witch)

7. Super Meat Boy. All of it. Ever.

Fuck those missions. Why aren't there more missions where I'm plundrin' booty from some random ass spanish merchant ships? You know, like a pirate? I mean, there are a few, but even then they had a ship based stealth mission in a swamp.

And if you really don't understand why it being possible to lose 10 minutes of progress in half a second is fucking annoying, you must have the patience of ten thousand tibetan monks.

they should've taken more advantage of the seamless ground to ship transition bit of gameplay. missions where I had to rush to my ship were always exhilarating compared to tailing missions haha

personally I thought about any big fights while playing through Mass Effect 2 and 3 on the hardest difficulty. come to think of it I'm at the last mission of ME3 and have yet mustered up the energy to complete that haha

ugh yeah thankfully that was optional loll...

I skipped a lot of side stuff on my Insanity playthrough of ME2 =/ I was just like sigh...nope

First time fighting Terrormorphus in Borderlands 2. I blew that horn and was like, "oh fuck... gunna need a bigger boat" :)

Also, the first time that bear showed up in Organ Trail. Nearly broke my mouse trying to shoot it so fast. Still came out alive, though.

Luvia Bloodmire's underground lab in Baldur's Gate Dark Alliance 2. Going through the first time is pretty simple. There are monsters, you kill the monsters, beat a boss, grab the loot and plot shard.

Only once you kill the boss Luvia floods the map with poison gas, unleashes /new/ monsters and you have to fight your way out again while your health steadily drains away. I could never beat it without cheating.
