Getting Veteran recommendation in COD4 . The 1st mission.
I didn't have too much trouble with that. In terms of being trainingit was simple, obviously as you said the difficult part was getting Veteran but this didn't exactly cause me problems.

Whilst not difficult I have to throw an annoyance in. Tomb Raider: The Last Revelation.
It was the first game without an optional training level at her house and you were required to play through this long training level that took you about an hour. Initially I found it quite interesting that they implimented the training into the game, but now I just find it annoying and unneccesary.

A lot of people have mentioned the GTA: San Andreas driving school. My personal hell was the barrel roll in the sports car, where you have to drive off a ramp, flip 360 degrees on the x axis and land perfectly on all four wheels, still facing exactly forwards.

The worst, though, was probably the original Driver. You had a time limit - 60 seconds, I think - to pull off this list of about 10 different moves while in an underground parking garage surrounded by cars and pillars. The best part was that you couldn't start the actual game until you finished that part. It was awful - why the hell would I need to have a donut completely mastered for a game where you drive around the city streets?
There was a dentist I used to go to with two gaming stations; a Nintendo 64 with Wave Race, and a Playstation with Driver. I went there for about 5 years, and the last time I went there it was still stuck in the training level.

I find that any Zelda game is complete agony now.
The story/tutorial bit at the beginning where you learn how to swing your sword etc etc is just a complete kick in the nuts.
I also found Shadowrun a really long process just to learn how to fire a gun -.-
