The robot that always talk to you in Portal. In the last 30 minutes or so when she starts saying she'll kill you i was pretty scared.
Holy crap that thing is freaky, and the area covered in writing from an escaped test subject, along with the turrets, make me shiver.
Oh God, her voice haunts me sometimes. Every time she spoke it made the little hairs on the back of my neck stand up. GLaDOS's voice always gives me super-heebiejeebies.

I would say that I would piss myself if Paxton Fettel was real.
I agree, one of the scariest characters from any game I've ever played was easily Paxton Fettel. If not Alma herself.
"props to the original F.E.A.R. script btw"
Yeah, I found Paxton much scarier. Mainly because he was real, not a ghost. And that scene on the roof where he hits you with a 2x4 and then you wake up and he's standing over you? Damn near pissed m'self!

Let's see...there's Paxton Fettel for previously mentioned reasons (got a very nice jump from the 2x4 scene), also from F.E.A.R. are the Project Icarus ninja things simply because of the impressive way they were introduced; hurling a corpse through two windows, running through the hallway and disappearing into the ceiling in the blink of an eye.

I also got a bit frightened of the player character in Saints Row 2, more as a "this is a guy you should not fuck with" kind of fear after the steps he went through to completely and utterly wreck Maero's life in The Brotherhood storyline, and the Texas Funeral you and Jonny Gat give Shogo Akuji after beating the crap out of him, while he's pleading for his life that he already knows is forfeit.

After that one cutscene I had to take a bit of a break from Saints Row 2.

Strange as it may seem, the thing that continues to give me willies is the shark that roams the waters in the Rusty Bucket Bay level for Banjo-Kazooie. I don't know why, its just the Jaws music starts playing and I'm swimming my ass off trying not to get hurt. I'm not hydrophobic or nothing, but I just can't play that level without my heart racing.

And I'd say the ReDeads from Wind Waker. The ones from OoT and Majora's Mask didn't scare me(I Always thought that it was Link screaming :p) but when you clearly saw the mouths move to the bloodcurdling scream from Wind Waker, and Link even shivering, that was about my response.

Del Lago from RE4 used to be a little creepy to me but then I started doing speed runs and that feeling passed.

And yeah, that eel from Mario 64. Strange, that's three water based enemies...seriously, I'm not hydrophobic, but I just don't like THOSE creatures.

I'd have to say the Witches from L4D because you hear the crying and then you get tense, you have to turn out your light so you can't see, and then when she does come after you she's either suddenly up and charging at you or you turn around she's right in your face preparing to claw your nipples off.

There's also the eel underwater from Super Mario 64. Something (I don't know what) about that thing scared me.

The bear from Condemned 2 was scary due to the fact that you can't fight him and the whole time I was running from it the only phrase out of my mouth was "Holy shit, holy shit, holy shit."

The Chainsaw guy from Resident Evil 4 was frightening because when you hear him rev that chainsaw you know he's nearby but you don't know where. I think the chainsaw sisters were not as frightening because you know that they're coming but the chainsaw guy is usually so sneaky that you don't know he's there until he's already sawing you into a bleeding, splattered mass of dead.

To this day, I still can't stand to play the hedge maze area in Resident Evil 4 because I can't stand when the dogs jump out of the bushes to gnaw at your genitals and tentacle rape you to death. Those tentacles and the sudden surprise attacks have always scared me.

The only other thing I can think of are the Smileys, Piggsy, The White Rabbit, and The Skinz in the Junkyard, all of which are in the game Manhunt. I'll explain why.

The Smileys: They talk all kinds of psychotic nonsense, wear smiley face masks with words such as die written on them, and they're just generally scary.

Piggsy: What's not scary? A fat guy in a pig mask with a chainsaw, sorta like Vincent Smith in the movie Motel Hell. Enough said.

The White Rabbit: People in dirty, creepy animal suits always scare me.

The Skinz: The always scared me when I was walking through the open junkyard because they would break the eerie silence with an ear-splitting scream that makes your heart skip a beat or two while you frantically search the darkness trying to find out if he's about to slice off your nipples or shove a knife (or bat) up your ass.

Sorry that it's so long but I wanted to get them all out there and explain why.

In any game that you get so focused and out of no where something just pops into view you didn't expect and scares the crap out of you. A few first person shooters did that, and many third persons.

Or Alma from F.E.A.R. Creepy little psycho girl.

ReDeads from LoZ: OoT scared the living shit out of me.
And Voldo from Soul Caliber.
Me and a friend have a contest going. We take turns doing to "Voldo Slide" as we call it in public. It's completely and utterly embarrassing, and just downright hilarious. Part of why I never found Voldo scary was because we could make fun of his primary method of transportation so easily. Otherwise, I'm sure I would feel different about him.

Tingle from Legend of Zelda.
I assume no explanation is needed.

Though on a serious note, those skinny zombie things in the sewers of Condemned. Those things kept popping out of nowhere and scaring the HELL out of me!

Death Claws in the Fallout series scare me most. Its just something about a nine foot tall nightmare with claws that cut through armour like a knife through butter that freaks me out.

Any of the townspeople from Call of Cthulu: Dark Corners of the Earth. Especially the cops.
I was also pretty scared of Velius from Final Fantasy Tactics as a kid.
OH! And any monster that lurks underwater, which you only see once you in the water (especially a particular Collossus).
Kinda like most of the underwater portion of the original Half-Life...every time I saw one of those underwater monsters my thoughts were as follows: "Oh my god, oh my god! get outta the fucking water!" as I scrambled frantically to the nearest ladder.

Also, the invisible bug things from RE4 messed me up pretty bad, especially in the sewer section. I mean, by the time you notice 'em they're already right next to you. Oh, and the Garden Maze in RE4 also.

Guess what, I got another one. When I was little, I borrowed Dino Crisis (first one) from my neighbor, the next day I returned it because I didn't even want that game in my house...The music and atmosphere of that game...*shivers* I still haven't beaten it to this day. (-_-;)

The Arrogant Man from the CHZO mythos games. His whispering and pulling you back to the Plane of Magic, and his following and teleportation and the blood and OH GOD MAKE IT STOP! I could barely play Trilby's Notes without screaming in horror.
