The whole rhaegar/lyanna thing comes from the Ned's flashback to the tower of joy, which involves three of the then kingsguard protecting a stronghold with no one other than lyanna supposedly inside, which would make sense if there was a royal son being born. And then ned mentions how she died in a bed full of blood, and he made some mysterious promise to her (that no one but howland reed knows of), then ned comes back to the north with a bastard son, completely out of character. It's really just a fan theory, but a lot of it seems to fit, and it's fairly popular.
